Pharmacological action
Sydnopharm is a peripheral vasodilator, an antianginal drug from the group of sydnonimine.
Expands peripheral venous vessels, has antiplatelet, analgesic and antianginal effects. Venodilating activity is due to the release after a series of metabolic transformations of nitric oxide (NO), which stimulates soluble guanylate cyclase, in this regard, molsidomine is considered as a NO donor. As a result of this, cGMP is accumulated, which contributes to the relaxation of the smooth muscles of the vessel wall (to a greater extent – veins). Reducing preload even without a direct effect on myocardial contractility leads to the restoration of the relationship between the myocardial oxygen demand and its intake, which is disturbed during coronary artery disease (oxygen demand is reduced by 26%). The drug relieves spasm of the coronary arteries, improves collateral circulation.
Increases exercise tolerance, reduces the number of angina attacks during exercise. Suppresses the early phase of platelet aggregation, reduces the excretion and synthesis of serotonin, thromboxane and other proaggregants.
In patients with chronic heart failure, reducing preload helps reduce the size of the heart chambers. It reduces the pressure in the pulmonary artery, reduces the filling of the left ventricle and the tension of the myocardial wall, stroke volume of blood.
The action begins 20 minutes after ingestion, reaches a maximum in 0.5-1 hours and lasts up to 6 hours. The development of tolerance with a decrease in effectiveness during long-term therapy is uncharacteristic (unlike nitrates).
Indications
Prevention of angina attacks.
Contraindications
– Acute myocardial infarction occurring with arterial hypotension.
– Severe arterial hypotension (systolic blood pressure below 100 mmHg).
– Collapse.
– Cardiogenic shock.
– Low left ventricular filling pressure.
– Toxic pulmonary edema.
– Concomitant use of sildenafil.
– Pregnancy.
– Lactation.
– Children and adolescents under 18 years of age (efficacy and safety not established).
– Hypersensitivity to the drug.
Precautions prescribe the drug in the following cases:
– Postponed hemorrhagic stroke.
– Increased intracranial pressure.
– Tendency to arterial hypotension.
– The elderly.
– Patients with glaucoma (especially angle-closure).
– In cases of cerebrovascular accident.
Use in pregnancy and lactation
Use of Sydnopharm is contraindicated in pregnancy. If necessary, the appointment of the drug during lactation should abandon breastfeeding.
Composition
1 tablet contains: molsidomine 2 mg.
Excipients: lactose, wheat starch, mannitol, colloidal silicon dioxide, magnesium stearate, hydroxypropyl methylcellulose, peppermint oil, dye Sikovit gob orange 8.5 (E110), microcrystalline cellulose.
Dosage and administration
The dosage regimen and duration of therapy are set individually. The drug is taken orally, regardless of food intake. The tablets are swallowed whole without chewing and drinking with a sufficient amount of liquid. The drug is prescribed in an initial dose of 1-2 mg (0.5-1 tab.) 2 times / day. At weekthe residual severity of the therapeutic effect, the frequency of administration is increased to 3 times / day. The maximum daily dose is 6 mg.
Side effects of
From the cardiovascular system: often – lowering blood pressure, collapse (with initially high blood pressure, the severity of its decrease is greater than with initially normal or low blood pressure) facial skin redness, tachycardia.
From the side of the central nervous system: headache (usually insignificant, disappears during further treatment), slowing down the rate of psychomotor reactions (to a greater extent at the beginning of treatment), dizziness.
From the digestive system: nausea, loss of appetite, diarrhea.
Allergic reactions: skin rash, itching, bronchospasm rarely – anaphylactic shock.
Drug Interaction
Simultaneous administration of Sidnopharm with vasodilator, antihypertensive agents and ethanol increases the severity of the antihypertensive effect. With the simultaneous use of Sidnopharma with acetylsalicylic antihypertensive agents and ethanol increases the severity of the antihypertensive effect. With the simultaneous use of Sidnopharma with acetylsalicylic antihypertensive agents and ethanol increases the severity of the antihypertensive effect. With the simultaneous use of Sidnopharma with acetylsalicylicthis acid increases its antiaggregant action.
Overdose
Symptoms: severe headache, severe BP, tachycardia.
Treatment: Symptomatic therapy is carried out if necessary.
Storage conditions
The drug should be stored in a dry, dark place at a temperature of no higher than 25 ° C.
