Pharmacological action

Clopixolum is an antipsychotic agent (antipsychotic), a derivative of thioxanthene.

Pharmacodynamics

The antipsychotic effects of antipsychotics are usually associated with blockade of dopamine receptors, which apparently causes a chain reaction, which involves other mediator systems.

Clopixol has a pronounced antipsychotic and specific inhibitory effect. Clopixol may have a transient, dose-dependent sedation. The rapid development of a sedative effect at the beginning of therapy (before the onset of antipsychotic action) is an advantage in the treatment of acute and subacute psychoses.

Tolerance to the nonspecific sedative effect of the drug occurs rapidly. The specific inhibitory effect of clopixol is especially beneficial in the treatment of patients with agitation, anxiety, hostility or aggressiveness.

Pharmacokinetics

Oral bioavailability of zuclopentixol is approximately 44%. The maximum concentration in blood serum is reached after 4 hours. Zuclopentixol slightly penetrates the placental barrier and in small quantities is excreted in breast milk.

Metabolites do not have antipsychotic activity and are excreted mainly with feces and, partially, with urine. The biological half-life is approximately 20 hours.

Indications

Acute and chronic schizophrenia and other psychotic disorders, especially with hallucinations, paranoid delusions and impaired thinking, as well as states of agitation, increased anxiety, hostility or aggressiveness.

Manic phase of manic-depressive psychosis.

Mental retardation combined with psychomotor agitation, agitation and other behavioral disorders.

Senile dementia, with paranoid ideas, confusion, disorientation, impaired behavior.

Contraindications

Acute poisoning with alcohol, barbiturates, opiates

Use during pregnancy and lactation

Special instructions

Clopixol may affect the ability to drive a car and other mechanisms. Therefore, at the beginning of therapy, caution must be exercised until the patient’s response to treatment is determined.

With prolonged therapy, careful monitoring of the patient’s condition is necessary.

With extreme caution, clopixol should be prescribed to patients with convulsive syndrome, chronic hepatitis, and cardiovascular disease.

Dosage and administration of

Doses of the drug should be selected individually depending on the patient’s condition. In general, small doses should be used initially, which then quickly build up to the optimum effective level depending on the clinical effect.

An acute attack of schizophrenia, other acute psychotic disorders, pronounced agitation and mania. Usually 10-50 mg / day.

In severe disorders and conditions of moderate severity, the initial dose of 20 mg / day may, if necessary, increase by 10-20 mg in 2-3 days to 75 mg per day or more.

Chronic psychotic conditions in schizophrenia and other chronic psychoses. Maintenance dose 20-40 mg / day.

Agitation in patients with oligophrenia. Usually 6-20 mg / day. If necessary, the dose may be increased to 25-40 mg / day.

Agitation and confusion in patients with senile disorders. Usually 2-6 mg / day (preferably given in the evening), if necessary, can be increased to 10-20 mg / day.

Side effects of the

From the nervous system: Extrapyramidal symptoms may develop, especially at the initial stage of treatment. In most cases, they are corrected by lower dosages and / or prescription of anti-Parkinsonian drugs. However, regular prophylactic use of the latter is not recommended.

In rare cases, with prolonged therapy, patients may develop tardive dyskinesia. Antiparkinsonian drugs do not eliminate its symptoms. Dosage reduction or, if possible, discontinuation of therapy is recommended.

From the side of mental activity: Drowsiness at the initial stage.

From the autonomic nervous system and cardiovascular system: Dry mouth, accommodation disturbance, urinary retention, constipation, tachycardia, orthostatic hypotension and dizziness.

From the liver: Rarely, minor transient changes in liver function tests are noted.

Drug Interactions

Chemically incompatible combinations not found. Clopixol may enhance the sedative effects of alcohol, barbiturates, and other central nervous system inhibitors.

Clopixol should not be prescribed together with guanethidine and similarly active agents, as antipsychotics can block their hypotensive effect.

Clopixol may decrease the effectiveness of levodopa and other adrenergic drugs, and a combination with metoclopramide and piperazine increases the risk of extrapyramidal symptoms.

Overdose

Symptoms: drowsiness, coma, extrapyramidal disorders, convulsions, hypotension, shock, hyper- or hypothermia.

Treatment: in case of ingestion it is necessary to wash the stomach as quickly as possible, the use of sorbent is recommended.

Subsequently, symptomatic and supportive therapy is performed.

Care should be taken aimed at maintaining the activity of the respiratory and cardiovascular systems.

Epinephrine (epinephrine) should not be used because this can lead to a further decrease in blood pressure.

Convulsions can be stopped with diazepam and extrapyramidal symptoms with biperiden.
