Indications
Depressive disorders of varying severity.
Contraindications
Absolute
– children and adolescents under 15
– hypersensitivity to

Relative
– simultaneous administration with adrenaline, norepinephrine, clonidine and its derivatives
– benign prostatic hyperplasia and obstruction of the urinary tract of a different genesis
– pregnancy
– lactation (breastfeeding).
Special instructions
With caution, prescribe the drug to patients with a history of seizures, with arterial hypertension, cardiomyopathy.
Ixel can be prescribed no earlier than 14 days after the withdrawal of MAO inhibitors. In addition, from the moment of discontinuation of the drug Xel to the start of therapy with MAO inhibitors, at least 7 days must elapse.
A gradual increase in the dose of the drug is recommended to reduce the severity of side effects.
When conducting local anesthesia with drugs containing adrenaline or norepinephrine during therapy with IxelВ® therapy, the dose of anesthetics should not exceed 0.1 mg per 10 min and 0.3 mg per 1 hour.
Do not drink alcohol during treatment with Ixel.
Influence on the ability to drive and control mechanisms
During the period of use of the drug should refrain from engaging in potentially hazardous activities that require increased attention and rapid psychomotor reactions.
Composition
1 caps. contains milnacipran hydrochloride 50 mg
Dosage and administration
The average daily dose is 100 mg, the drug should be taken in 2 doses in the morning. Depending on the severity of symptoms, the dose may be increased to 250 mg.
The drug is preferably taken with food.
Duration of therapy is set individually.
In patients with renal failure, a dose reduction is necessary depending on the creatinine clearance values.
Side effects
From the central nervous system: a sense of anxiety, dizziness rarely – tremor.
From the digestive system: rarely – dry mouth, nausea, vomiting, constipation in isolated cases – a moderate increase in transaminase activity.
Other: rarely – palpitations, increased sweating, hot flashes, difficulty urinating in isolated cases – serotonergic syndrome.
Side effects are rare, mainly during the first 2 weeks of therapy, usually stop on their own as regression of depressive symptoms and do not require discontinuation of the drug.
Drug interaction
With the simultaneous use of Ixel with MAO inhibitors, sumatriptan, lithium preparations, there is a risk of developing serotonergic syndrome.
The simultaneous use of Ixel with adrenaline and norepinephrine can provoke the development of a hypertensive crisis and cardiac arrhythmias.
When taken with digoxin (especially with parenteral administration), there is a risk of potentiation of hemodynamic disorders.
With the simultaneous use of Ixel inhibits the hypotensive effect of clonidine and its derivatives.
Overdose
Symptoms: if you accidentally increase your dose – nausea, vomiting, sweating, constipation. When taking the drug in a dose exceeding 800-1000 mg – vomiting, difficulty breathing, tachycardia. After taking at an excessively high dose (1900-2800 mg milnacipran) in combination with other psychotropic drugs (most often benzodiazepines), drowsiness, hypercapnia, and disorders of consciousness are added to the symptoms described above.
Treatment: gastric lavage, symptomatic therapy. Medical monitoring of the patient’s condition for at least 24 hours is recommended. There is no specific antidote.
