Pharmacological action
MOMAT – local anti-inflammatory, glucocorticoid, antipruritic.
Pharmacodynamics
Mometasone is a synthetic GCS with anti-inflammatory, antipruritic, and antiexudative effects. GCS induce the release of proteins that inhibit phospholipase A2 and commonly known as lipocortins, which control the biosynthesis of inflammatory mediators such as PG and RT by inhibiting the release of their common precursor, arachidonic acid.
Pharmacokinetics
Absorption is negligible. 8 hours after a single application to intact skin (without an occlusive dressing), about 0.4% (cream) and 0.7% (ointment) of the active substance are found in the systemic circulation. Mometasone is extensively metabolized in the liver, excreted mainly by the kidneys and in small quantities with bile. T1 / 2 is approximately 5.8 hours (ointment).
Indications
– psoriasis, atopic dermatitis and other dermatoses, including in the scalp
– allergic rhinitis, seasonal and chronic (treatment and prevention).
Contraindications
hypersensitivity to any component of the drug or corticosteroids
rosacea, perioral dermatitis
bacterial, viral (Herpes simplex, chickenpox, Herpes zoster) or fungal skin infection
tuberculosis, syphilis skin areas long-term treatment)
lactation (use in large doses and / or for a long time)
children under 2 years of age (ointment).
Precautions: application to the skin of the face and intertriginous skin, the use of occlusive dressings, as well as the treatment of large areas of skin and / or prolonged treatment (especially in children).
Pregnancy and lactation
The safety of mometasone furoate during pregnancy and lactation has not been studied. GCS penetrate the placental barrier. Long-term treatment and the use of large doses during pregnancy should be avoided due to the threat of negative effects on the development of the fetus.
GCS is excreted in breast milk. In the case when it is supposed to use GCS in large doses and / or for a long time, breast-feeding should be stopped.

Dosage and administration
Externally, apply a thin layer to the affected skin once a day.
The duration of the course of treatment is determined by its effectiveness, as well as patient tolerance, the presence and severity of side effects.
Side effects
Rarely – skin irritation and dryness, burning sensation, itching, folliculitis, hypertrichosis, acne, hypopigmentation, perioral dermatitis, allergic contact dermatitis, skin maceration, secondary infection, signs of atrophy of the skin, striae,. In less than 1% of cases – the formation of papules, pustules.
When applying external forms of corticosteroids for a long time and / or for treating large areas of the skin, or using occlusive dressings, especially in children and adolescents, side effects may occur, characteristic for GCS systemic effects, including adrenal insufficiency and Cushing’s syndrome.
Overdose
Symptoms: suppression of hypothalamic-pituitary-adrenal function, including secondary adrenal insufficiency.
Treatment: symptomatic, if necessary – correction of electrolyte imbalance, withdrawal of the drug (with long-term therapy – gradual withdrawal).
