Pharmacological action

Antiarrhythmic, replenishing deficiency of magnesium and potassium.

Panangin is a source of potassium and magnesium ions. Prevents or eliminates hypokalemia. Improves myocardial metabolism. Improves cardiac glycoside tolerance. It has antiarrhythmic activity.

Asparaginate carries potassium and magnesium ions and facilitates their penetration into the intracellular space.

Entering the cells, asparaginate is included in the metabolic processes. Magnesium ion plays an important role in maintaining potassium and calcium homeostasis, has the properties of a calcium channel blocker, and is involved in many enzymatic reactions, protein and carbohydrate metabolism.

Indications

Heart failure, myocardial infarction, cardiac arrhythmias (mainly ventricular arrhythmias), additional

therapy in the treatment of cardiac glycosides, hypokalemia, hypomagnesemia, insufficient intake of magnesium and potassium ions in the body when they are deficient in the diet.

Contraindications

Hypersensitivity, acute and chronic renal failure, Addison’s disease, anuria, AV block II-III degree, cardiogenic shock, severe arterial hypotension, hyperkalemia, hypermagnesemia, severe myasthenia gravis, oliguria hemolysis,

Special instructions

Caution is advised to prescribe Panangin to patients with an increased risk of developing hyperkalemia. In this case, it is necessary to regularly monitor the level of potassium ions in the blood plasma.

Before taking the drug, the patient should consult a doctor.

With rapid on / in the introduction of the drug may develop hyperemia of the skin.

Effect on the ability to drive vehicles and control mechanisms

The drug does not affect the ability to drive a car and engage in activities that require an increased concentration of attention and speed of psychomotor reactions.

Dosage and administration of

For angina pectoris (as an adjuvant) Panangin is administered orally in 2 tablets 3 times / day daily.

In case of cardiac arrhythmias, Panangin is administered intravenously slowly or dropwise at 2 amp. / day (morning and evening).

For prophylactic purposes or with maintenance therapy, Panangin is prescribed 1 tablet 3 times / day.

Duration of therapy is set individually.

Side effects

From the nervous system and sensory organs: fatigue, hyporeflexia, dizziness, sensation of heat, paresthesia.

From the cardiovascular system and blood (hematopoiesis, hemostasis): lowering blood pressure, ECG changes, impaired myocardial conduction and cardiac rhythm, phlebitis, vascular spasms, vasodilation, vein thrombosis.

From the respiratory system: dyspnea.

From the digestive tract: diarrhea, nausea, vomiting, dry mouth, rarely – abdominal pain, bleeding and ulceration of the intestines and gastric mucosa, perforation of the intestinal wall, flatulence, ulcer of the esophagus and duodenum.

From the side of metabolism: hyperkalemia.

From the musculoskeletal system: muscle weakness.

From the skin: itching, excessive sweating.

Drug Interactions

Inhibits the absorption of oral forms of tetracycline, iron salts and sodium fluoride (a 3-hour interval between doses is required). Due to the presence of potassium in the drug, the risk of developing hyperkalemia increases (control of potassium level in blood plasma is necessary) when combined with ACE inhibitors, beta-blockers, cyclosporine, potassium-sparing diuretics, heparin and NSAIDs, and the inhibitory effect on intestinal motility increases. With the development of hyperkalemia, a decrease in the effect of digitalis preparations is possible.

Due to the presence of magnesium in the drug, neuromuscular blockade is enhanced when combined with antidepolarizing muscle relaxants, CNS depression – when combined with drugs for anesthesia. Reduces the effectiveness of neomycin, polymyxin B, tetracycline and streptomycin. Calcium preparations decrease effectiveness, and calcitriol helps increase the level of magnesium in blood plasma.
