Drug Interaction

Stimulants of microsomal oxidation in the liver (phenytoin, ethanol, barbiturates, rifampicin, phenylbutazone, tricyclic antidepressants) increase the production of hydroxylated active metabolites of paracetamol, which makes it possible to develop severe intoxication with small overdoses.

Inhibitors of microsomal oxidation (cimetidine) reduce the risk of hepatotoxic action of the drug.

Paracetamol enhances the effects of MAO inhibitors, sedatives, ethanol.

Ethanol promotes acute pancreatitis.

Antidepressants, antiparkinsonian, antipsychotic drugs, phenothiazine derivatives increase the risk of urinary retention, dry mouth, constipation.

ACS increase the risk of developing glaucoma.

Phenylephrine reduces the hypotensive effect of guanethidine.

Guanetidine enhances alpha-adrenostimulating effect, and tricyclic antidepressants have the sympathomimetic effects of phenylephrine.

The drug reduces the effectiveness of uricosuric drugs.

Slow gastric emptying (propantheline) may cause a delayed onset of the drug.

With acceleration of gastric emptying (metoclopramide), the drug begins to act faster.

The drug can enhance the effect of indirect anticoagulants.

Overdose

An overdose of paracetamol appears after taking 10-15 g (15-20 bags of Prostudox).

Symptoms: epigastric discomfort, nausea, vomiting, paleness of the skin, decreased appetite, nausea, vomiting, hepatonecrosis (the severity of necrosis directly depends on the degree of overdose), increased activity of hepatic transaminases, increase in prothrombosis time48 (48) ), a detailed clinical picture of liver damage manifests itself in 1-6 days. Rarely, impaired liver function develops rapidly and may be complicated by renal failure (renal tubular necrosis).

Treatment: symptomatic, gastric lavage, administration of SH-group donors and precursors of glutathione-methionine synthesis 8-9 h after overdose and N-acetylcysteine after 12 h. Need for additional therapeutic measures (further administration of methionine, N / – acetylcysteine) is determined depending on the concentration of paracetamol in the blood, as well as the time elapsed after its administration.
