Pharmacological action

Supirocin is a broad-spectrum antibiotic. Mupirocin is an antibiotic produced by the microorganism Pseudomonas fluorescens. Mupirocin inhibits bacterial cell protein synthesis through reversible binding to isoleucyl – transport RNA – synthetase, which prevents the inclusion of isoleucine in protein chains under construction.

When used in minimal inhibitory concentrations, mupirocin has a bacteriostatic effect, and at higher concentrations it has a bactericidal effect. Due to the specific mechanism of action and the unique chemical structure, there is no cross-resistance with other antibiotics.

With proper use of mupirocin, the risk of the emergence of resistant strains of microorganisms is small.

In vitro antibacterial spectrum: Gram – positive aerobes: Staphylococcus aureus (including strains resistant to methicillin and producing beta-lactomas), Staphylococcus epidermidis, Staphylococcus speciese, Coagulase-negative Staphylococcus.

Gram – negative anaerobes: Haemophilus influenzae, Neisseria gonorrhoeae, Neisseria meningitidi, Branhamella catarrhalis, Pasteurella multocida, Proteus mirabilis, Proteus vulgaris,

Enterobacter cloacae, Entarobacterterundund aerogenella.

Insensitive: Corynebacterium species, Enterobacteriaceae, Gram-negative non-fermentative bacilli, Micrococcus.

Pharmacokinetics

Mupirocin practically does not penetrate intact skin. In the case of adsorption through damaged skin, it is metabolized to a microbiologically inactive metabolite – monic acid – and is rapidly excreted by the kidneys.

Indications

Primary and secondary infectious skin lesions caused by microorganisms sensitive to mupirocin, including: primary skin infections: – impetigo,

– folliculitis,

– furunculosis (including the external auditory canal furuncle, and

secondary infections:

– infected eczema,

– infected injuries (abrasions, insect bites, wounds, burns).

Contraindications

Hypersensitivity to any component of the drug Supirocin, children up to 3 years of age.

Precautions: Macrogol – polyethylene glycol – can be absorbed through the wound surface or through other damage to the skin and excreted by the kidneys.

Macrogol ointments cannot be used on large surfaces in conditions accompanied by increased absorption of polyethylene glycol, and especially in moderate and severe renal failure.

Pregnancy and lactation

There are currently insufficient data on the use of mupirocin in pregnancy, therefore, the use of the drug is possible if the expected benefit exceeds the possible risk of complications.

Excretion of the drug in breast milk is unlikely, however, the use of the drug during lactation is possible if the expected benefit exceeds the possible risk of complications.

Composition

100 g of ointment for topical and external use contains:

active substances:

mupirocin – 2 g.

excipients:

macrogol 400 (polyethylene glycol 400),

macrogol 4000 (polyethylene glycol 4000).

Dosage and administration

A small amount of Supirocin is applied to the affected skin surface.

A dressing may be applied on top. After applying the drug to the skin, wash your hands thoroughly. Apply 2-3 times a day for up to 10 days, depending on the severity of the lesion.

In the absence of a clinical effect within 3 to 5 days, it is recommended that the diagnosis and treatment tactics be reviewed.

Side effects

Supirocin ointment is well tolerated.

Very rarely there is a burning sensation, itching at the site of application of the drug.

In less than 1% of cases, erythema, dry skin, swelling, contact dermatitis. Skin allergic reactions (edema at the site of application, rash) to mupirocin or ointment base were rarely observed.

When applying Supirocin ointment in isolated cases, generalized allergic reactions have been reported.

Drug Interaction

Not reported.

Supirocin ointment cannot be combined with other topical preparations because of the risk of dilution, which is manifested by a decrease in antibacterial activity and a potential loss of stability.

Storage Conditions

In a dry, dark place at a temperature not exceeding 25 ° C. (do not freeze).
