Pharmacological action of

Acetylsalicylic acid has antipyretic and anti-inflammatory effects, reduces pain, especially caused by the inflammatory process, and also moderately inhibits platelet aggregation and thrombosis, improves microcirculation in the focus of inflammation.

Caffeine increases the reflex excitability of the spinal cord, stimulates the respiratory and vasomotor centers, dilates the blood vessels of the skeletal muscles, brain, heart, kidneys, reduces platelet aggregation, reduces drowsiness, and a feeling of tiredness. In this combination, caffeine in a small dose practically does not have a stimulating effect on the central nervous system, but it helps to regulate the tone of brain vessels.

Paracetamol has an analgesic, antipyretic and mild anti-inflammatory effect, due to its effect on the thermoregulation center in the hypothalamus and a mild ability to inhibit prostaglandin (Rd) synthesis in peripheral tissues.

Indications

Pain syndrome of mild to moderate severity (of various origins): headache, migraine, toothache, neuralgia, myalgia, arthralgia, algodismenorea. Feverish syndrome: in acute respiratory diseases (ARI), flu.

Contraindications

– Peptic ulcer of the stomach and duodenum in the exacerbation phase

– Gastrointestinal bleeding (including history)

– Severe impaired liver and / or kidney function, gout surgic interventions accompanied by heavy bleeding

– Pregnancy (I and III trimester), lactation period

– Deficiency of glucose-6-phosphate dehydrogenase

– Glaucoma

– Hypersensitivity to the drug, asthma, induced by acetylsalicylic acid salicylates and other non-steroidal anti-inflammatory drugs

– Children’s age (up to 15 years – risk of developing Reye’s syndrome in children with hyperthermia due to viral diseases)

Precautions

Renal failure of mild to moderate degree, liver failure with increased levels of transaminases, benign hyperbilirubinemia (including Gilbert syndrome, alcoholic liver disease), alcoholism, epilepsy and susceptibility to seizures, advanced age, gout, hypertension.

Use during pregnancy and lactation

Contraindicated in pregnancy (I and III trimesters), during lactation.

Acetylsalicylic acid has a teratogenic effect when used in the first trimester of pregnancy leads to a malformation – splitting of the upper palate in the third trimester – to inhibition of labor activity (inhibition of prostaglandin synthesis), to closure of the arterial duct in the fetus, which causes pulmonary vascular hyperplasia and vascular hypertension pulmonary circulation.

Is excreted in breast milk, which increases the risk of bleeding in a child due to impaired platelet function.

Composition

Active substances: acetylsalicylic acid – 240 mg, paracetamol – 180 mg, caffeine monohydrate or caffeine (anhydrous caffeine) -30 mg.

Excipients: cocoa – 14 mg, citric acid – 5 mg, potato starch – 63 mg, low molecular weight povidone (low molecular weight polyvinylpyrrolidone) – 4 mg, talc – 9 mg, calcium stearate-5 mg.

Dosage and administration of

The drug should not be taken for more than 5 days as an analgesic drug and more than 3 days as an antipyretic (without prescription and medical supervision).

Inside (during or after meals), 1 tablet every 4 hours, with pain – 1-2 tablets, the average daily dose is 3-4 tablets, the maximum daily dose is 8 tablets.

The course of treatment is no more than 3-5 days.

Side effects of

Adverse reactions characteristic of acetylsalicylic acid, paracetamol and caffeine can be observed: anorexia, nausea, vomiting, gastralgia, decreased platelet aggregation, erosive and ulcerative lesions of the gastrointestinal tract and duodenal ulcer, gastrointestinal tract bronchospasm erythema multiforme exudative (including Stevens-Johnson syndrome), toxic epidermal necrolysis (Lyell’s syndrome), renal failure, liver failure, dizziness, tachycardia, increased blood pressure. With prolonged use, headache, visual impairment, tinnitus, hypocoagulation, hemorrhagic syndrome (nosebleeds, gum bleeding, purpura, etc.), kidney damage with papillary necrosis, Reye syndrome in children.

Drug Interaction

Increases the effect of heparin, indirect anticoagulants, reserpine, steroid hormones and hypoglycemic drugs.

Concomitant use with other nonsteroidal anti-inflammatory drugs, methotrexate increases the risk of side effects.

Decreases the effectiveness of spironolactone, furosemide, antihypertensive drugs, and anti-gout drugs that promote uric acid.

Barbiturates, rifampicin, salicylamide, antiepileptic drugs and other stimulants of microsomal oxidation contribute to the formation of toxic metabolites of paracetamol affecting liver function.

Metocopramide accelerates the absorption of paracetamol.

Under the influence of paracetamol, the elimination time of chloramphenicol is increased 5-fold. Re-administration of paracetamol may enhance the action of anticoagulants (dicumarin derivatives). Caffeine accelerates the absorption of ergotamine.

At the same time taking the drug and alcohol-containing liquids increases the risk of toxic liver damage.

Overdose

Do not exceed the recommended dose and duration of administration!

Symptoms:

– with mild intoxication: nausea, vomiting, stomach ache, dizziness, ringing in the ears

– with severe intoxication: inhibition, drowsiness, collapse, convulsions, difficulty breathing, anuria.

Treatment: gastric lavage using activated charcoal, symptomatic therapy, depending on the state of metabolism – the introduction of sodium bicarbonate, sodium citrate or sodium lactate, which increases the excretion of acetylsalicylic acid by alkaline urination.
