Indications

Therapy for depression of various etiologies and structures in adults.

Treatment for panic disorder with / without agoraphobia.

Treatment of obsessive-compulsive disorder (OCD).

Contraindications

Cipramil is contraindicated in case of hypersensitivity to citalopram or any of the excipients.

The drug should not be used in combination with monoamine oxidase inhibitors (MAOs), including selegiline, and also within 14 days after stopping their use. Treatment with MAO inhibitors can be started no earlier than 7 days after cessation of cipramil.

Cipramil should not be prescribed to pregnant and breast-feeding women if the potential clinical benefit does not prevail over the theoretical risk, because the safety of the drug during pregnancy and lactation in humans has not been established.

Use during pregnancy and lactation

Contraindicated. Nursing mothers should stop breastfeeding.

Special instructions

The risk of suicide is inherent to depression and can persist until significant improvement, occurring spontaneously or as a result of ongoing therapy.

Careful monitoring of patients undergoing antidepressant treatment, especially at the beginning of treatment, is necessary because of the potential for clinical impairment and / or suicidal manifestations (thoughts and behavior). This precaution should be observed in the treatment of other mental disorders because of the possibility of a simultaneous illness with a depressive episode.

The use of SSRIs in the third trimester of pregnancy can adversely affect the psychophysical development of the newborn. The following disorders were registered in newborns whose mothers took SSRIs until delivery: irritability, tremor, hypertension, increased muscle tone, constant crying, difficulty in sucking, and poor sleep.

Violations may indicate serotonergic effects or withdrawal symptoms. In the case of using SSRIs during pregnancy, their intake should not be abruptly interrupted.

Cipramil should be used with caution in individuals whose activities are related to mechanisms or the management of moving assets. Cipramil does not reduce the intellectual ability and speed of psychomotor reactions.

However, it can be expected that patients who are prescribed treatment with CIPRAMIL have a certain decrease in attention and concentration due to an existing disease, treatment, or both.

Dosage and administration of

Cipramil is prescribed once a day.The drug can be used at any time of the day, regardless of the meal.

Depression: Depression therapy begins with 20 mg of Cipramil per day. Depending on the individual reaction of the patient and the severity of depression, the dose can be increased to a maximum of 60 mg per day.

Panic disorder: during the first week of treatment, a dose of 10 mg of Cipramil per day is recommended, then the dose rises to 20 mg per day. The daily dose, depending on the individual reaction of the patient, can be further increased. The maximum dose is 60 mg / day.

Obsessive-compulsive disorder: The initial dose of Cipramil is 20 mg per day. If necessary, the dose may be increased. The maximum dose is 60 mg per day.

Elderly patients (over 65 years old). The recommended daily dose for the elderly is 20 mg. Depending on the individual reaction and the severity of depression, the dose can be increased to a maximum of 40 mg per day.

In case of liver failure, the drug should be limited to the minimum recommended doses. The maximum dose is 30 mg per day.

Dose adjustment is not required for mild to moderate renal failure.

Side effects

Undesirable effects of CYPRAMILA are transient in nature and are weakly expressed. They are observed during the first one to two weeks of treatment and usually significantly weaken as the patient’s condition improves.

Perhaps the appearance of dry mouth, nausea, drowsiness, weakness, excessive sweating, tremors, agitation, insomnia, diarrhea, sexual disorders.

CYPRAMIL may cause a slight decrease in heart rate, which usually does not have clinical significance. However, in patients with an initially low heart rate, this can lead to bradycardia.

In exceptional cases, when using large doses of the drug, convulsive seizures may develop. Very rarely hyponatremia, manifestations of serotonin syndrome, withdrawal syndrome, extrapyramidal disorders, purpura can occur.

With phase inversion and the development of the manic state, Cipramil should be canceled.

Drug Interactions

Chemically incompatible combinations not found.

The simultaneous use of CYPRAMIL and MAO inhibitors can cause hypertensive crisis (serotonin syndrome).

CYPRAMIL may enhance the effects of sumatriptan and other serotonergic drugs.

With the simultaneous use of cimetidine causes a moderate increase in the equilibrium concentration of cipramil in the blood. Therefore, caution is advised when prescribing maximum doses of CIPRAMIL simultaneously with the use of high doses of cimetidine.

No interaction of CYPRAMIL with lithium and alcohol. However, the simultaneous use of CYPRAMIL and alcohol is not recommended.

No clinically significant pharmacokinetic interaction with phenothiazines and tricyclic antidepressants has been established.

In clinical studies, no pharmacodynamic interaction of Cipramil and benzodiazepines, antipsychotics, analgesics, lithium, antihistamines, antihypertensive drugs, beta-blockers and other cardiotropic drugs.

Overdose

Symptoms. The following symptoms and signs may occur: drowsiness, coma, articulation disorders, episodes of major convulsions (grand mal), sinus tachycardia, sometimes nodular rhythm, sweating, nausea, vomiting, cyanosis, hyperventilation. The clinical picture is not characteristic.

Treatment. There is no specific antidote. Treatment is symptomatic and supportive. Following oral administration of CIPRAMIL, gastric lavage should be performed as soon as possible. Medical monitoring is recommended.
