Pharmacological action

Antiplatelet agent, angioprotector, immunomodulator. It has an inhibitory effect on platelet aggregation, improves microcirculation.

It has a vasodilating effect, reduces the resistance of resistive coronary vessels (mainly arterioles), increases the volumetric velocity of coronary blood flow. Able to reduce perfusion of ischemic myocardial zones.

The mechanism of action is not fully understood.

It is believed that dipyridamole increases the adenosine content (disrupting its reuptake), and also promotes an increase in cAMP concentration due to inhibition of the PDE enzyme.

As a pyrimidine derivative, dipyridamole is an inducer of interferon and has a modulating effect on the functional activity of the interferon system, increases the reduced production of interferon alpha and interferon gamma by leukocytes in vitro.

Increases non-specific resistance to viral infections.

Indications

Treatment and prevention of cerebrovascular accidents according to the ischemic type, dyscirculatory encephalopathy.

Prevention of arterial and venous thrombosis and their complications, prevention of thromboembolism after surgery for prosthetic heart valves.

Prevention of placental insufficiency in complicated pregnancy.

As part of complex therapy for any disorders of microcirculation.

As an inducer of interferon and an immunomodulator for the prevention and treatment of influenza, SARS.

Contraindications

· Acute myocardial infarction, unstable angina pectoris, common stenotic atherosclerosis of the coronary arteries.

Special instructions

To reduce dyspeptic symptoms, take with milk.

Dosage and administration of

Unless otherwise prescribed by a doctor, the following dosage regimens are recommended:

To reduce platelet aggregation, it is recommended to use Dipyridimol at a dose of 75–225 mg / day in several doses. In severe cases, the dose can be increased to 600 mg / day.

For the prevention of thromboembolic syndrome, stent thrombosis and coronary artery bypass grafts – on the first day, 50 mg with ASA, then 100 mg, frequency of administration – 4 times a day (canceled 7 days after surgery, subject to continued administration of ASA at a dose of 325 mg / day) or 100 mg 4 times a day for 2 days before surgery and 100 mg 1 hour after surgery (if necessary in combination with warfarin).

For the prevention of influenza and other acute respiratory diseases, especially during epidemics, take as follows: 50 mg / day 1 time in 7 days for 4-5 weeks.

For the prevention of relapse in patients often suffering from respiratory viral infections, Dipyridamole is recommended to be taken according to the scheme: 100 mg / day (2 times 50 mg with an interval of 2 hours) 1 time per week for 8 to 10 weeks.

Dipyridamole is recommended to be taken on an empty stomach, without chewing coated tablets, with a small amount of liquid.

Side effects

Palpitations, bradycardia, thrombocytopenia, changes in the functional properties of platelets, bleeding, sensation of stuffy ear, noise in the head, arthritis. It may cause vomiting, diarrhea, epigastric pain, as well as symptoms such as weakness, dizziness, nausea, headache, myalgia, rhinitis. Typically, these side effects disappear with prolonged use of dipyridamole.

As a result of the potential vasodilating effect, dipyridamole in high doses can cause arterial hypotension, a sensation of “hot flashes” and tachycardia, and coronary robbery syndrome (at doses greater than 225 mg per day).

Hypersensitivity reactions like rash or urticaria are possible.

If side effects occur, you should inform your doctor.

Drug interactions

Xanthine derivatives (coffee, tea) can weaken the vasodilator and antithrombotic action of dipyridamole.

The use of dipyridamole simultaneously with direct and indirect anticoagulants (heparin) and thrombolytics or acetylsalicylic acid increases the risk of hemorrhagic complications, which must be taken into account when used together.

Dipyridamole may enhance the effects of antihypertensive drugs.

Dipyridamole may weaken the anticholinergic properties of cholinesterase inhibitors.

The antiplatelet effect of antibiotics of cephalosporins (cefamandol, cefaperazone, cefatetan), penicillin antibiotics, tetracyclines, chloramphenicol, beta-lactams, nicotinic acid is enhanced.

Antacids reduce maximum concentration due to reduced absorption.

Overdose

Symptoms: short-term hypotension.

Treatment: symptomatic (including management of vasopressor agents).
