Description
Pharmacological action
Competitive m-cholinergic inhibitor. After oral administration, fesoterodine is rapidly and intensively hydrolyzed with the participation of nonspecific esterases with the formation of the active metabolite 5-hydroxymethyl tolterodine, which determines the m-anticholinergic activity of fesoterodine.
Activation of the postganglionic parasympathetic m-cholinergic receptors of the smooth muscles of the bladder induces detrusor contraction. Fesoterodine inhibits these receptors in the bladder, which is believed to lead to the development of appropriate pharmacological effects.
Clinical use of fesoterodine has been shown to increase bladder volume at the time of the first detrusor contraction and increase bladder capacity. These effects are enhanced in proportion to the dose increase.
Indications
Treatment of overactive bladder in the presence of symptoms of acute urinary incontinence with the need for urgent and frequent urination.
Contraindications
Urinary retention, delayed evacuation of food from the stomach, uncontrolled angle-closure glaucoma, hypersensitivity to fesoterodine.
Composition
1 tab. fesoterodine 4 mg
Dosage and administration
Take orally.
The recommended starting dose is 4 mg 1 time / day. Depending on the effectiveness and tolerability, the dose can be increased to 8 mg 1 time / day.
Do not exceed the dose of 4 mg / day in patients with impaired renal function with a CC <30 ml / min in patients receiving strong inhibitors of the CYP3A4 isoenzyme, such as ketoconazole, itraconazole and clarithromycin. It is not recommended to use fesoterodine in patients with impaired liver function (class C on the Child-Pugh scale).
