Pharmacological action

Beta-adrenergic stimulant, with selectivity for beta2-adrenergic receptors of the smooth muscles of the bronchi, has a bronchodilator effect. Foradil is effective for the prevention and relief of bronchospasm caused by allergens, physical activity, histamine, methacholine (diagnostic test). Foradil has tocolytic properties. The onset of action is 1-3 minutes after inhalation, the duration of the bronchodilator effect is 12 hours.

Indications

Relief and prevention of reversible airway obstruction – COPD, bronchial asthma (including atopic and physical activity asthma).

Contraindications

Hypersensitivity, childhood (up to 5 years). With caution. Diabetes mellitus, severe CHF, IHD, heart rhythm disturbances, AV block III stage, lengthening of the QT interval (QT adjusted> 0.44 s), GOKMP, thyrotoxicosis, pregnancy, lactation.

Special instructions

Exercise extreme caution when using Foradil (especially in terms of dose reduction) and careful monitoring of patients are required in the presence of the following concomitant diseases: coronary heart disease and cardiac conduction, especially grade III AV block severe heart failure idiopathic subvalvular aortic stenosis hypertrophic obstructive cardiomyopathy thyrotoxicosis known or suspected prolongation of the QTc interval (QT corrected)> 0.44 sec.

Due to the hyperglycemic effect inherent to beta2-adrenergic agonists, in patients with diabetes, additional regular monitoring of blood glucose levels is recommended.

Usually, patients with asthma who require beta2-adrenergic agonist therapy should also receive regular doses of anti-inflammatory drugs (for example, corticosteroids for inhalation or for oral and / or sodium cromoglycate in children). When prescribing the drug to patients not receiving anti-inflammatory therapy, it should be started simultaneously with the use of Foradil. When prescribing Foradil, it is necessary to assess the condition of patients in relation to the adequacy of the anti-inflammatory therapy that they receive. After starting treatment with Foradil, patients should be advised to continue anti-inflammatory therapy without changes, even if improvement is noted.

A physician needs to review the basic treatment of bronchial asthma if asthma symptoms persist while using Foradil, or if the amount of Foradil required to control the symptoms of the disease increases, as this usually indicates a worsening of the course of the disease.

The consequence of beta2-adrenergic agonist therapy may be the development of potentially serious hypokalemia. Because this effect of the drug can be enhanced by hypoxia and concomitant treatment, special care should be taken in patients with severe bronchial asthma. In these cases, regular monitoring of serum potassium levels is recommended.

As with other inhalation therapy, the possibility of developing paradoxical bronchospasm should be considered. If it occurs, immediately discontinue the drug and prescribe an alternative treatment.

Dosage and administration of

Foradil is administered by inhalation. Drug Interactions

Foradil (like other beta2-adrenostimulants) should be used with caution in patients receiving drugs such as quinidine, disopyramide, procainamide, phenothiazines, antihistamines, MAO inhibitors, and other tricyclic antidepressants it is known that they lengthen the QT interval, because in these cases, the effect of adrenostimulants on the cardiovascular system may be enhanced. When using drugs that can extend the QT interval, the risk of ventricular arrhythmias increases.

The simultaneous use of other sympathomimetic agents can lead to aggravation of side effects of Foradil.

The simultaneous use of xanthine derivatives, corticosteroids, or diuretics can enhance the potential hypokalemic effect of beta2-adrenergic agonists. Hypokalemia may increase the predisposition to the development of cardiac arrhythmias in patients receiving digitalis preparations.

Beta-blockers may weaken the effect of Foradil. In this regard, Foradil should not be used in conjunction with beta-blockers (including eye drops), unless any emergency causes are required to use such a combination of drugs.
