Pharmacological action of

GCS for topical use in ophthalmology.

When applied topically, hydrocortisone acetate suppresses the inflammatory and allergic reactions that occur in the outer shells and anterior segment of the eye.

The action of hydrocortisone acetate, like other corticosteroids, is based on the suppression of the function of leukocytes and tissue macrophages. It limits the migration of leukocytes, macrophages and mast cells into the area of inflammation and inhibits the formation of interleukin-1. Helps stabilize lysosomal membranes, thereby reducing the concentration of proteolytic enzymes in the area of inflammation. Reduces capillary permeability due to histamine release. Suppresses fibroblast activity and collagen formation. It inhibits the activity of phospholipase A2, which leads to the suppression of the synthesis of prostaglandins and leukotrienes.

Pharmacokinetics

Hydrocortisone acetate may penetrate through the cornea into the moisture of the anterior chamber due to lipophilic properties. The penetration of hydrocortisone acetate into the tissue depends on the condition of the cornea and increases significantly in case of inflammation or damage to the mucous membrane of the eye.

Use during pregnancy and lactation

To date, there are no clinical data regarding the use of the drug Hydrocortisone-PIC during pregnancy and lactation.

It is possible to use the drug Hydrocortisone-PIC for the treatment of pregnant and lactating mothers as prescribed by the attending physician, if the expected therapeutic effect exceeds the risk of possible side effects. The duration of the drug in this category of patients is no more than 7-10 days.

Composition

1 g of eye ointment contains:

Active ingredient: hydrocortisone acetate 10 mg

Excipients: white petrolatum, liquid paraffin, lanolin.

Dosage and administration

A strip of ointment 1 cm long is laid over the lower eyelid 2-3 times a day.

With the simultaneous use of several ophthalmic medicines, it is recommended to lay Hydrocortisone-PIC in the form of an eye ointment no earlier than 15 minutes after using eye drops.

The duration of therapy with Hydrocortisone-PIC usually does not exceed 2-3 weeks.

How to use

For application, slightly tilt your head back, put the ointment in the lower conjunctival sac, then close your eyes. During the application of an eye ointment, the tube should not be in contact with the surface of the skin or conjunctiva. After application, close the tube tightly.

Side effects

From the side of the organ of vision: very rarely – burning sensation, scleral injection, contact dermatitis, dermatoconjunctivitis, eczema of the eyelids. With prolonged use of the drug, secondary glaucoma and complicated cataracts may develop.

Drug Interaction

With the simultaneous use of Hydrocortisone-PIC and other agents that may cause an increase in intraocular pressure (eg, atropine and other cholinergic drugs), an additional increase in intraocular pressure may occur, especially in patients predisposed to it.
