Pharmacological action
Ferretab comp. – a tool for the treatment and prevention of iron deficiency conditions.
The action is determined by the properties of the active ingredients in its composition.
Iron fumarate is a salt of iron, a trace element necessary for the synthesis of hemoglobin. When using iron in the form of salts, it quickly replenishes its deficiency in the body, which leads to a gradual regression of clinical (weakness, fatigue, dizziness, tachycardia, soreness and dryness of the skin) and laboratory symptoms of anemia.
Folic acid is necessary for the normal maturation of megaloblasts and the formation of normoblasts. It stimulates erythropoiesis, participates in the synthesis of amino acids, nucleic acids, purines and pyrimidines, in the exchange of choline. During pregnancy, protects the body from the action of teratogenic factors.
The drug helps maintain and restore normal levels of iron in the blood, and also prevents such complications, such as anemia, miscarriage, early childbirth and impaired mental development of the child, which can occur with a lack of iron and folic acid in the body. The amount of assimilable iron varies between 5 and 35%.
In serum, iron binds to transferrin, is involved in the formation of hemoglobin, myoglobin, cytochrome oxidase, catalase and peroxidase, or is stored in body tissues as ferritin.
Pharmacokinetics
The excretion of iron occurs with feces, urine and sweat.
Folic acid is absorbed mainly in the upper intestine (duodenum) and binds to proteins by 64%. It is metabolized in the liver and excreted mainly through the kidneys, as well as partially with feces.
Indications
Treatment and prevention of iron deficiency conditions (pregnancy, malabsorption of iron in the digestive tract, prolonged bleeding, unbalanced and malnutrition).
Contraindications
Hypersensitivity to the components of the drug.
Severe cardiovascular or cerebrovascular disease.
Arterial hypertension in combination with ischemic heart disease, aortic aneurysm, atrioventricular block I-III degree, arrhythmia.
tachycardia.
Violation of tear production.
Prematurity children under 6 years old (with accommodation spasm).
Hyperthyroidism.
Hepatic porphyria.
Congenital deficiency of glucose-6-phosphate dehydrogenase.
Rhinitis.
Caution
Diabetes mellitus (risk of high blood pressure associated with impaired autonomic regulation).
Concomitant use with monoamine oxidase inhibitors (including within 21 days after cessation of their use).
Sickle cell anemia, wearing contact lenses after surgery (decreased healing due to conjunctival hypoxia). pernicious anemia (vitamin B12 deficiency)
anemia not caused by a lack of iron or folic acid.
Use during pregnancy and lactation
Use is possible if indicated.
Special instructions
Before starting iron therapy, it is necessary to conduct a study of the level of serum iron, serum ferritin (i.e. document iron deficiency).
When taking the drug, dark staining of the feces is possible, the benzidine test is false positive.
Composition
1 capsule contains: active ingredient: (1 capsule contains 3 mini-tablets of iron fumarate and 1 mini-tablet of folic acid), iron fumarate 163.56 mg, folic acid 0.54 mg,
excipients: lactose monohydrate – 34.47 mg silicon colloidal dioxide – 2.3 mg magnesium stearate – 2.3 mg polyacrylate (30% dispersion) – 13.98 mg MCC – 24.25 mg.
Dosage and Administration
Ferretab Comp. taken orally, on an empty stomach, drinking plenty of fluids, 1 capsule per day. With a pronounced lack of iron or folic acid, you need to increase the dose to 2-3 capsules per day. Maintenance treatment should be continued for at least 4 weeks after reaching normal hemoglobin levels to normalize serum ferritin, which reflects iron stores in the body.
Side effects
Usually well tolerated. In some cases, a temporary adverse effect on the gastrointestinal tract is possible – a feeling of fullness and discomfort in the stomach, nausea, vomiting, allergic reactions, constipation.
Drug Interactions
Iron supplementation worsens when taken with antacids.
Avoid co-administration with tetracycline antibiotics due to impaired absorption of iron.
Cholestyramine, phenytoin, phenobarbital, carbamazepine, sulfasalazine, hormonal contraceptives, folic acid antagonists, trimethoprim, triamteren, solid foods, bread, raw cereals, dairy products, eggs, tea – reduce iron absorption.
Overdose
Symptoms: increased manifestations of side effects.
Treatment: A study of serum iron and serum ferritin levels. In case of significant excess of normal serum ferritin values, an iron chelator (desferal) should be prescribed in accordance with the dose recommended by the manufacturer.
