Pharmacological action

Epicurus – antiulcer drug. Specific proton pump inhibitor (H + -K + -ATPase).

Metabolized in parietal cells of the gastric mucosa to active sulfonamide derivatives that inhibit sulfhydryl groups of H + -K + -ATPase. It blocks the final stage of hydrochloric acid synthesis, reducing basal and stimulated secretion, regardless of the nature of the stimulus.

The use of the drug in a dose of 30 mg causes a decrease in gastric secretion of hydrochloric acid by 80-97%. The inhibitory effect increases over 4 days. After discontinuation of the drug, acidity remains below 50% of the basal level for 39 hours, no ricocheting increase in secretion is noted.

Secretory activity is restored 3-4 days after taking the drug. The drug does not affect the motility of the digestive tract.

In patients with Zollinger-Ellison syndrome, the drug lasts longer.

Indications

Peptic ulcer of the stomach and duodenum (in the acute phase), erosive-ulcerative esophagitis, reflux esophagitis, Zollinger-Ellison syndrome, stress ulcers of the gastrointestinal tract eradication Helicobacter pylori (as part of combination therapy), non-ulcer dyspepsia.

Contraindications

Hypersensitivity, pregnancy (I trimester), lactation, malignant neoplasms of the gastrointestinal tract.

Precautions: liver failure, pregnancy (II-III trimesters), old age, age under 18 years.

Use during pregnancy and lactation

The drug is contraindicated for use in the first trimester of pregnancy and during lactation (breastfeeding). In the II and III trimesters of pregnancy, the drug is prescribed with caution.

Dosage and Administration

Capsules should be taken orally, swallowed whole, without chewing.

With peptic ulcer of the duodenum in the exacerbation phase, appoint30 mg / day for 2-4 weeks (in resistant cases – up to 60 mg / day).

In case of gastric ulcer in the acute phase and in case of erosive-ulcerative esophagitis, 30-60 mg / day is prescribed for 4-8 weeks.

With erosive and ulcerative lesions of the gastrointestinal tract caused by NSAIDs, 30 mg / day for 4-8 weeks.

For eradication of Helicobacter pylori – 30 mg 2 times a day for 10-14 days in combination with antibacterial therapy.

For anti-relapse treatment of peptic ulcer of the stomach and duodenum – 30 mg / day.

For anti-relapse treatment of reflux esophagitis – 30 mg / day for a long time (up to 6 months).

In Zollinger-Ellison syndrome, the dose is selected individually until basal secretion level is reached

Side effects

From the central nervous system: headache rarely – malaise, dizziness, drowsiness, depression, anxiety.

From the digestive system: decreased or increased appetite, nausea, abdominal pain rarely – diarrhea, constipation in isolated cases – ulcerative colitis, gastrointestinal candidiasis, increased activity of liver transaminases, hyperbilirubinemia.

From the respiratory system: rarely – cough, pharyngitis, rhinitis, upper respiratory tract infection, flu-like syndrome.

From the hemopoietic system: rarely – thrombocytopenia (with hemorrhagic manifestations) in some cases – anemia.

Allergic reactions: skin rash in some cases – photosensitivity, erythema multiforme.

Other: extremely rare – myalgia, alopecia.

Drug Interaction

Slows the elimination of drugs metabolized in the liver by microsomal oxidation (including diazepam, phenytoin, indirect anticoagulants). Reduces theophylline clearance by 10%. Changes the pH-dependent absorption of drugs belonging to the groups of weak acids (deceleration) and bases (acceleration). Compatible with ibuprofen, indomethacin, diazepam, propranolol, warfarin, oral contraceptives, phenytoin, prednisolone. Sucralfate reduces bioavailability by 30% (the interval between these drugs for 30-40 min must be observed). Antacids slow and reduce absorption (they should be administered 1 h before or 1-2 h after lansoprazole).
