Pharmacological action

Antiepileptic drug, mechanism of action unclear, does not alter normal synaptic transmission.

Effective in both focal and major epilepsy (epileptiform manifestations, photoparoxysmal reaction).

A stable state is achieved after 48 hours when taking the drug 2 times a day.

Indications

Epilepsy (focal epileptic seizures with or without transition to secondary large seizures) – as part of complex therapy.

Contraindications

Hypersensitivity (including to other pyrrolidone derivatives), lactation, age up to 4 years.

Caution. Pregnancy.

Special instructions

Should not be used during pregnancy unless absolutely necessary (given that interruptions in anticonvulsant therapy can lead to exacerbation of the disease, harmful to the mother and the fetus).

The drug, like other antiepileptic drugs, should be discontinued gradually (for example, a dose reduction of 500 mg when taken 2 times a day every 2-4 weeks).

During the treatment period, care must be taken when driving vehicles and engaging in other potentially hazardous activities that require an increased concentration of attention and speed of psychomotor reactions.

Composition

Levetiracetam 1000 mg.

Dosage and administration

Inside, with plenty of fluids, regardless of food intake, 2 times a day.

Adults and adolescents over 16 years of age, the initial dose is 500 mg 2 times a day, from the first day of treatment. Depending on the clinical response and tolerance, the daily dose may be increased to 1,500 mg 2 times a day (a 500 mg dose increase is possible every 2-4 weeks).

Individual dose selection is recommended for older people and patients with chronic renal failure. With normal renal function and CC more than 80 ml / min, the recommended dose and frequency of administration is from 500 to 1500 mg 2 times a day, with mild renal failure and CC 50-79 ml / min – from 0.5 to 1 g 2 times a day with an average degree of renal failure and KK of 30-49 ml / min – from 250 to 750 mg 2 times a day, with a severe degree of renal failure and KK less than 30 ml / min – from 250 to 500 mg 2 times a day.

At the terminal stage of chronic renal failure – from 0.5 to 1 g once a day, after dialysis, an additional dose of 250-500 mg is recommended. Dialysis patients on the first day of treatment with levetiracetam, a saturating dose of 750 mg is recommended.

In severe violations of the liver and CC less than 70 ml / min – a 50% reduction in the daily intake.

Patient interaction

possible combined application with other antiepileptic drugs (phenytoin, carbamazepine, valproic acid, phenobarbital, gabapentin and primidone), with oral contraceptive drugs (ethinyl estradiol and levonorgestrel.) – at a daily dose of levetiracetam to 1 g of digoxin and warfarin – with a daily dose of levetiracetam up to 2 g.

Overdose

Symptoms: there is no evidence of an overdose of the drug (at doses exceeding 5 g / day).

Treatment: after an acute overdose – gastric lavage, there is no special antidote, hemodialysis, symptomatic therapy is possible.
