Pharmacological action

Logimax – a drug that has antiarrhythmic, antihypertensive and antianginal effects. When using Logimax, blood pressure stably and effectively decreases within 24 hours, that is, the entire interval between doses. It enhances the blood flow in the coronary system, increases the supply of blood to the ischemic zones of the myocardium without developing a decrease in blood supply to the brain areas, activates the functioning of blood circulation bypasses, and reduces peripheral vascular resistance. Also, increased blood pressure decreases during physical exertion, at rest and stress.

Contraindications

Hypersensitivity, acute myocardial infarction, unstable angina pectoris, severe sinus bradycardia, AV block II-III degree, sinus node weakness syndrome, severe peripheral circulation disorders, decompensated heart failure, cardiogenic shock.

Use during pregnancy and lactation

Contraindicated in pregnancy. At the time of treatment should stop breastfeeding.

Special instructions

Caution should be exercised when combined with cardiac glycosides. Logimax ® may increase the symptoms of peripheral circulatory disorders, for example, “intermittent” lameness. Particular care is required in the following conditions: severe renal failure (CC less than 30 ml / min), aortic stenosis, impaired liver function, myocardial infarction complicated by acute heart failure, arterial hypotension, which in susceptible patients can cause myocardial ischemia, acute metabolic acidosis.

It is not recommended to start treatment with Logimax ® in patients who have not previously received antihypertensive therapy.

It is not recommended to prescribe the drug to patients with latent or revealed heart failure with clinical manifestations without appropriate therapy.

The use of Logimax ® may affect carbohydrate metabolism or mask hypoglycemia, however, with the use of Logimax ®, the risk of exposure to carbohydrate metabolism is lower than with non-selective beta-blockers.

In some cases, existing violations of AV conduction can be amplified, leading to AV blockade.

Patients taking Logimax ® should not be given intravenous BMKK type verapamil.

In patients with Prinzmetal angina, the frequency and severity of angina attacks may increase due to coronary spasm caused by stimulation of alpha-adrenergic receptors. In this regard, it is not recommended to prescribe non-selective beta-blockers in this group of patients. Selective beta1-blockers should be used with caution.

In patients with bronchial asthma or chronic obstructive pulmonary disease, concomitant therapy with beta2-adrenergic agonists is necessary. In the case when the patient begins to take Logimax ®, an increase in the dose of beta2-adrenergic agonists may be required.

With beta-blockers, anaphylactic shock can take a more severe form. The use of epinephrine (adrenaline) in a commonly used dose does not always lead to the expected therapeutic effect.

It is recommended that patients with pheochromocytoma be prescribed alpha-blockers in parallel with Logimax ®.

The combined use of drugs that induce the isoenzyme of CYP3A4 leads to a significant decrease in the concentration of felodipine in the blood plasma and insufficient therapeutic effect from taking the drug (see the section ² њInteraction with other drugs and other types of interaction ² ќ). The combined use of such drugs should be avoided.

The combined use of drugs that inhibit the CYP3A4 isoenzyme leads to a significant increase in the concentration of felodipine in blood plasma.

Logimax ® with grapefruit juice should be avoided due to a significant increase in the concentration of felodipine in blood plasma.

Avoid abrupt withdrawal of beta-blockers, especially in patients with a high risk of cardiovascular complications, due to the possible increase in symptoms of chronic heart failure and an increased risk of myocardial infarction and sudden death. If necessary, the withdrawal of the drug Logimax ® should be carried out gradually by reducing the dose within 1-2 weeks. In case of surgery, the anesthetist should be warned that the patient is taking Logimax ®.

It is not recommended to discontinue treatment with beta-blockers in patients who have surgery. In case of surgical intervention, the anesthesiologist should be warned about the received therapy with Logimax ®.

Cases of hyperplasia of the mucous membrane of the tongue and gums after taking felodipine in patients with severe gingivitis / periodontitis have been reported. This side effect can be avoided or reduced by showing good oral hygiene.

It is recommended that patients with pheochromocytoma be given alpha-blockers at the same time.

Effect on the ability to drive a car and work with

equipment During the treatment with Logimax ®, care must be taken when driving vehicles and engaging in other potentially dangerous activities that require increased concentration of attention and speed of psychomotor reactions (dizziness and fatigue may occur).

Composition

Active ingredients: felodipine 5 mg metoprolol succinate (as 50 mg metoprolol tartrate) 47.5 mg

Excipients: ethyl cellulose hydroxypropyl methyl cellulose hydroxypropyl cellulose lactose anhydrous polyoxyl 40 hydrogenated castor oil MCC paraffin polyethylene glycol sodium silicate aluminum dihydrate sodium silicate sodium dihydrate (E172).

Dosage and administration

Inside, in the morning, without chewing, washed down with water. The initial dose is 1 tablet. Once a day, if necessary, double the dose.

There is no need to adjust the treatment regimen in patients with impaired renal function. Usually, there is no need to adjust the treatment regimen in patients with impaired liver function, including in patients with cirrhosis of the liver. In the presence of symptoms of a serious liver dysfunction (e.g. in patients in the history of which there were operations with bypass surgery) should consider the possibility of reducing the dose of the drug.

In elderly patients, 1 tablet is usually sufficient. Once a day, if necessary – 2 times a day.

Drug Interactions

Verapamil-type calcium channel blockers potentiate a negative foreign and chronotropic effect. Enhances the negative foreign and dromotropic effect of antiarrhythmic drugs (quinidine analogues, amiodarone), cardiodepressive – inhaled anesthetics. Increase the concentration of metoprolol in plasma cimetidine, hydralazine, alcohol, lowers rifampicin. Indomethacin and other prostaglandin synthetase inhibitors weaken the hypotensive effect.

Overdose

Symptoms: hypotension, sinus bradycardia, AV blockade, heart failure, cardiogenic shock, cardiac arrest, bronchospasm, nausea, vomiting, cyanosis. The first signs appear in 20 min – 2 h after reception.

Treatment: induction of vomiting, gastric lavage symptomatic therapy: in case of severe hypotension, bradycardia or threat of heart failure – beta1-agonist (in / in at intervals of 2-5 min or infusion) or dopamine (in / in) or atropine sulfate for the purpose of blockage of the vagus nerve dobutamine, norepinephrine, glucagon (1-10 mg).
