Pharmacological action

Antianginal. Nitrospray-ICN expands veins and arteries, reduces pre- and afterload, OPSS. Improves coronary blood flow and redistributes it in favor of ischemic areas of the myocardium, reduces heart function. Inhibits platelet aggregation and adhesion.

Indications

Angina pectoris (relief and prevention of seizures), acute myocardial infarction and acute left ventricular failure (complex therapy).

Contraindications

Hypersensitivity, shock, collapse, acute myocardial infarction with low left ventricular filling pressure, arterial hypotension (below 90 mm Hg), hypertrophic cardiomyopathy, hemorrhagic stroke, head injury, toxic lung edema, closed pulmonary edema,

Special instructions

Care must be taken when abruptly moving to an upright position from a lying or sitting position, prolonged standing, physical activity, or hot weather (orthostatic reactions may develop).

Composition of

Nitroglycerin solution 4% – 250 g of

Dosage and administration of

Nitrospray-ICN is taken sublingually, in a sitting position, against the background of breath holding. Stopping an attack of angina pectoris: 1-2 doses containing 0.4-0.8 mg of nitroglycerin (if necessary, repeatedly after at least 30 s), a maximum of 3 doses at intervals of 5 minutes. Prevention of angina attacks: 1 dose 5-10 minutes before the expected load or stress. The maximum single dose is 1.6 mg (4 doses), daily – 6.4 mg (16 doses).

Side effects

From the nervous system and sensory organs: headache, dizziness, weakness, blurred vision, motor anxiety. On the part of the cardiovascular system and blood (blood formation, hemostasis): arterial hypotension, reflex tachycardia, increased symptoms of angina pectoris (rarely). On the part of the digestive tract: nausea, vomiting, dry mouth. On the part of the skin : facial flushing, dermatitis, rash.

Drug interaction

Other antihypertensive drugs, antidepressants, PDE inhibitors (sildenafil), ethanol can enhance the hypotensive effect of nitrospray. At the same time, the effectiveness of Nitrospray may decrease.

With simultaneous use with Nitrospray, a decrease in the anticoagulant effect of heparin is possible.

Overdose

Symptoms: cyanosis of the lips, nails, palms, dizziness, fainting, sensation of intense pressure in the head, shortness of breath, unusual tiredness or weakness, weak and rapid heartbeat, high body temperature, cramps.

Treatment: moving the patient to supine position with raised legs, symptomatic and supportive therapy.
