Pharmacological action of

Pharmacodynamics

Pancreatinum is a powder from pig pancreas. Pancreatic enzymes that make up pancreatin have a proteolytic, amylolytic and lipolytic effect and contribute to the breakdown of proteins, fats, carbohydrates, improve the gastrointestinal tract, and normalize digestion.

Pharmacokinetics

Mezim tablets are coated with an acid-resistant coating that does not dissolve under the action of hydrochloric acid of the stomach and, thus, protects the enzymes contained in the preparation from inactivation. Dissolution of the membrane and the release of enzymes occurs at pH close to neutral or slightly alkaline.

Indications

– Lack of exocrine pancreatic function, (chronic pancreatitis, cystic fibrosis)

– chronic inflammatory and dystrophic diseases of the stomach, intestines, liver, gall bladder. Conditions after resection or irradiation of these organs, accompanied by impaired digestion of poverty, flatulence, diarrhea (as part of combination therapy)

– to improve digestion, write in patients with normal gastrointestinal function in case of nutrition errors

– preparation for x-ray and ultrasound examination of the abdominal organs.

Contraindications

– Hypersensitivity to the components of the

preparation – acute pancreatitis or chronic pancreatitis in the acute stage. Nonetheless, the appointment is acceptable in the phase of fading exacerbation when the diet expands, if there are signs of digestion

– hereditary galactose intolerance, lactase deficiency or glucose-galactose malabsorption syndrome

– children under 3 years of age (for this dosage form).

Use during pregnancy and lactation

The safety of pancreatin during pregnancy has not been adequately studied.

Use is possible in cases where the expected benefit to the mother outweighs the potential risk to the fetus.

Composition

1 tablet contains:

Active substances:

pancreatin with minimal enzymatic activity:

lipase 3500 UNITS Ph.Eur

amylase 4200 UNITS Ph.Eur

protease 250 UNITS Ph.Eur.

Excipients:

microcrystalline cellulose,

sodium carboxymethyl starch (type A),

silicon dioxide colloidal,

magnesium stearate.

Shell composition:

talcum powder, hypromellose, azorubic varnish (E122), simethicone emulsion 30% (dry weight), polyacrylate dispersion 30% (dry weight), titanium dioxide (E171), macrogol 6000.

Dosage and administration

Dose is determined individually depending on the degree of digestion.

Adults: 1-2 tablets. before meals, without chewing and drinking with water. If necessary, an additional 1-4 tab.

In children: the drug is used as directed by a doctor.

The duration of treatment can vary from a few days (if the digestive process is disturbed due to errors in the diet) to several months or years (if constant replacement therapy is necessary).

Side effects

Allergic reactions are possible, with prolonged use in high doses, the development of hyperuricosuria is possible.

In case of cystic fibrosis, if the required dose of pancreatin is exceeded, strictures (fibrotic colononaty) may develop in the ileocecal region and in the ascending colon.

Drug Interaction

When administered with pancreatin-containing drugs, folic acid absorption may be reduced.

The action of sugar-lowering drugs (acarbose, miglitol) may be diminished when co-administered with pancreatin.

With the simultaneous use of pancreatin with iron, it is possible to reduce the absorption of the latter.

Concomitant use of antacids containing calcium carbonate and / or magnesium hydroxide can reduce the effectiveness of the drug.

Overdose

There are no data on overdose and intoxication with the drug.

Possible: hyperuricosuria, hyperuricemia, constipation in children.

Treatment: withdrawal of the drug, symptomatic therapy.
