Pharmacological action

Myotropic antispasmodic. It inhibits PDE, causes the accumulation of cAMP in the cell and reduces the content of intracellular calcium. Reduces the tone of the smooth muscles of internal organs (gastrointestinal tract, respiratory, urinary, reproductive system) and blood vessels. It causes the expansion of arteries, helps to increase blood flow, including cerebral. It has a hypotensive effect.

In high doses, it reduces the excitability of the heart muscle and slows down intracardiac conduction.

When used in medium therapeutic doses, the effect on the central nervous system is weak.

Pharmacokinetics

Bioavailability is 54%. Binding to plasma proteins – 90%. It is well distributed in the body, penetrates histohematological barriers. Metabolized in the liver.

T1 / 2 – 0.5-2 hours. Possible increase to 24 hours.

It is excreted by the kidneys in the form of metabolites. Completely removed from the blood during dialysis.

Indications

Spasms of the smooth muscles of the abdominal organs, bronchi, peripheral vessels, cerebral vessels, kidneys, angina pectoris (as part of combination therapy).

As an adjuvant for sedation.

Contraindications

AV block, glaucoma, severe liver failure, old age (risk of hyperthermia), children under 6 months of age, hypersensitivity to papaverine.

Special instructions

Use with caution in conditions after traumatic brain injury, in chronic renal failure, with insufficiency of adrenal gland function, hypothyroidism, prostatic hyperplasia, supraventricular tachycardia, shock conditions.

IV should be administered slowly and under medical supervision.

Alcohol use should be excluded during treatment.

Composition

1 ml contains: Active substances: papaverine hydrochloride – 20 mg.

Excipients: water d / i – up to 1 ml, disodium edetate dihydrate – 0.05 mg, methionine – 0.1 mg.

Dosage and administration

Inside – 40-60 mg 3-5 times / day. Rectally – 20-40 mg 2-3 times / day.

When i / m, s / c or iv, a single dose for adults is 10-20 mg, the interval between administrations is at least 4 hours. For elderly patients, the initial single dose is not more than 10 mg. For children aged 1 year to 12 years, the maximum single dose is 200-300 mcg / kg.

Side effects

Possible: nausea, constipation, drowsiness, excessive sweating, arterial hypotension, increased activity of hepatic transaminases.
