Drug Interactions

Stimulators of microsomal oxidation in the liver (phenytoin, ethanol, barbiturates, rifampicin, phenylbutazone, tricyclic antidepressants) increase the production of hydroxylated active metabolites of paracetamol, which makes it possible to develop severe intoxications with small intoxications.

Microsomal oxidation inhibitors (cimetidine) reduce the risk of hepatotoxicity.

Paracetamol enhances the effects of MAO inhibitors, sedatives, ethanol.

Ethanol contributes to the development of acute pancreatitis.

Antidepressants, antiparkinsonian, antipsychotic drugs, phenothiazine derivatives increase the risk of urinary retention, dry mouth, and constipation.

corticosteroids increase the risk of developing glaucoma.

Phenylephrine reduces the hypotensive effect of guanethidine.

Guanethidine enhances the alpha adrenostimulating effect, and tricyclic antidepressants enhance the sympathomimetic effects of phenylephrine.

The drug reduces the effectiveness of uricosuric drugs.

When delaying gastric emptying (propantheline), a slow onset of the drug may occur.

When accelerating the emptying of the stomach (metoclopramide), the drug begins to act faster.

The drug may enhance the effect of indirect anticoagulants.

Overdose

An overdose of paracetamol is apparent after taking 10-15 g (15-20 bags of Prostudox).

Symptoms: epigastric discomfort, nausea, vomiting, paleness of the skin, decreased appetite, nausea, vomiting, hepatonecrosis (the severity of necrosis directly depends on the degree of overdose), increased activity of hepatic transaminases, increase in prothrombosis time48 (48) ), a detailed clinical picture of liver damage manifests itself in 1-6 days. Rarely, impaired liver function develops rapidly and may be complicated by renal failure (renal tubular necrosis).

Treatment: symptomatic, gastric lavage, administration of SH-group donors and precursors for glutathione-methionine synthesis 8-9 h after overdose and N-acetylcysteine after 12 h. The need for additional therapeutic measures (further administration of methionine, in / in the introduction of N-acetylcysteine) is determined by concentration paracetamol in the blood, as well as the time elapsed after its administration.
