Pharmacological action
Erinit – a venous vasodilator, has an antianginal effect. Stimulates the formation of nitric oxide in the vessel wall, which is an endothelial relaxing factor. Erinitis reduces preload (expansion of peripheral veins and a decrease in blood flow to the right atrium) and afterload (OPSS) on the heart reduces myocardial oxygen demand. It restores blood flow in small endocardial vessels, improves in the epicardial coronary arteries, increases exercise tolerance in patients with coronary artery disease, angina pectoris. Reduces pressure in the pulmonary circulation. Compared to nitroglycerin, Erinit has a slower, but longer lasting effect (up to 10 hours). When taken orally, the onset of the effect – after 30-45 minutes (with sublingual – after 15-20 minutes), duration – 4-5 hours.
Indications
ischemic heart disease, angina pectoris (prophylaxis, including in the post-infarction period) heart failure (as part of complex therapy).
Contraindications
Hypersensitivity, severe arterial hypotension (systolic blood pressure below 90 mmHg, diastolic blood pressure below 60 mmHg), collapse, shock, acute myocardial infarction, with hypotension, hemorrhagic cerebral trauma, angle-closure glaucoma, severe liver failure (risk of methemoglobinemia), severe renal failure, severe anemia, hypertrophic obstructive cardiomyopathy, constrictive pericarditis, there heart pond severe stenosis of the aortic and / or mitral valve, hyperthyroidism, primary pulmonary hypertension (hyperemia of insufficiently ventilated alveolar zones can lead to hypoxia), chronic heart failure with low left ventricular filling pressure, concomitant use of phosphodiesterase-5 inhibitors (including sildenafil, vardenafil, tadalafil), lactose and sucrose intolerance, deficiency of sucrose-isomaltase, lactase or glucose-galactose malabsorption, hypersensitivity to the drug and its components, other organic nitrates, under 18 years of age (efficacy and safety not established).
Caution
Applied with a tendency to hypotension in the elderly, dehydrated patients with a tendency to orthostatic hypotension in history.
Use during pregnancy and lactation
Use of the drug during pregnancy and lactation is contraindicated.
Composition
1 tablet contains 10 mg pentaerythritol tetranitrate.
Dosage and administration of
Erinit is taken orally, 20 mg 3-4 times a day, if necessary up to 40 mg 4 times a day, 80 mg 2 times a day is acceptable. If necessary, sublingual administration is possible. When used in combination with beta-adrenergic blockers and BMCC, it is possible to prescribe 1 time per day (at a time when angina attacks occur more often).
Side effects
Allergic reactions, headache, dizziness, nausea, diarrhea, weakness, tachycardia, increased fatigue, anxiety, increased sweating, decreased blood pressure, orthostatic hypotension.
Overdose
Tolerance may develop with high doses.
Symptoms: orthostatic hypotension, sensation of head constriction, pulsating headache, severe sweating, skin cold, covered with sticky sweat, or hyperemic, heartbeat, bradycardia, conduction disturbance (blockade), shortness of breath, Kussmaul’s breathing, bloody diarrhea, vomiting, dizziness, confusion, syncopal states, coma, fever, paralysis, visual impairment.
Treatment: gastric lavage, horizontal positioning with elevated legs, with severe hypotension and / or in patients in shock – fluid injection into / into drip infusion solutions, monitoring of methemoglobin level in the blood, with its elevation dose of 1 g inside, methylthioninium chloride (Methylene blue) in / in up to 50 ml of 1% solution, toluidine blue – 2 – 4 mg / kg body weight in / in quickly, if necessary – again in a dose of 2 mg / kg at intervals of 1 h, oxygen therapy, hemodial with, obmennoe perelyvanye blood.
