Pharmacological action
A drug that improves microcirculation. An angioprotector derived from methylxanthine.
The mechanism of action of pentoxifylline is associated with inhibition of phosphodiesterase and the accumulation of cAMP in the smooth muscle cells of blood vessels, in the formed elements of the blood, in other tissues and organs.
It inhibits the aggregation of platelets and red blood cells, increases their flexibility, reduces the increased concentration of fibrinogen in the plasma and enhances fibrinolysis, which reduces blood viscosity and improves its rheological properties. It has a weak myotropic vasodilator effect, slightly reduces OPSS and has a positive inotropic effect. When using the drug, there is an improvement in microcirculation and oxygen supply to the tissues, to the greatest extent in the limbs and central nervous system, and to a moderate extent in the kidneys. The drug slightly dilates the coronary vessels.

Special instructions
Careful monitoring is necessary in patients with severe cardiac arrhythmias (risk of worsening arrhythmias), with arterial hypotension (risk of further decrease in blood pressure), impaired renal function in patients with CC less than 30 ml / min (risk of cumulation and increased risk of side effects ), pronounced impaired liver function (risk of cumulation and an increased risk of side effects) and an increased tendency to bleeding, for example, with the use of anticoagulants or with disorders in the coagulation system blood (risk of more severe bleeding).
Caution should be exercised when prescribing Trental to patients with gastric and duodenal ulcer in the history of patients recently undergoing surgery (increased risk of bleeding, and therefore requires systematic monitoring of hemoglobin and hematocrit).
In patients with arterial hypotension and labile circulation, parenteral administration of Trental is prescribed gradually, since a decrease in blood pressure (up to collapse) is possible, and in some cases, angina attacks.
Patients with heart failure need appropriate treatment to compensate for blood circulation. In this case, the introduction of a large volume of liquids should be avoided.
In patients with impaired renal function, the dosage regimen should be selected individually.
Composition
1 tablet contains:

Dosage and administration
The average dose of the drug for oral administration is 400 mg (1 tab.) 2-3 times / day. The maximum daily dose is 1200 mg (3 tablets).
Take the whole pill, without chewing, during or immediately after a meal, with plenty of water.
In patients with severe renal impairment (CC less than 30 ml / min), the dose can be reduced to 400-800 mg / day.
In patients with liver failure, the dose of the drug is selected individually.
Side effects of
In cases of using Trental 400 in large doses, hot flashes can occur (redness of the face or burning sensation, such as burning sensation, pressure and overflow in the stomach, nausea, vomiting, diarrhea is very rare – heart rhythm disturbances (tachycardia).
Allergic reactions: sometimes – itching, flushing of the skin, urticaria, rarely – severe anaphylactic / anaphylactoid reactions with the development of angioedema, bronchospasm and even shock.
From the side of the central nervous system: possibly – dizziness, headache, agitation, sleep disturbance very rarely – cases of the development of aseptic meningitis.
From the digestive system: very rarely – increased activity of hepatic transaminases, intrahepatic cholestasis.
From the cardiovascular system: very rarely – angina attacks, tachycardia, arterial hypotension.
From the blood coagulation system: very rarely – thrombocytopenia in patients with an increased tendency to bleeding – bleeding (for example, from blood vessels of the skin, mucous membranes, stomach, intestines).
Drug interaction
When used together, Trental potentiates the action of a number of antihypertensive and hypoglycemic agents (such as insulin, and oral hypoglycemic drugs).
The simultaneous use of pentoxifylline and theophylline can lead to an increase in theophylline levels in the blood and increase its side effects.
Overdose
Symptoms: nausea, dizziness, tachycardia, decreased blood pressure, arrhythmia, hyperemia of the skin, chills, loss of consciousness, areflexia, tonic-clonic convulsions.
Treatment: Symptomatic therapy is carried out if necessary. The patient should be given a horizontal position with legs raised. The specific antidote is unknown. They monitor the vital functions of the body and general activities aimed at maintaining them, monitor the patency of the respiratory tract in convulsions – diazepam.
