Pharmacological action

Thyroxine synthesis regulator – iodine preparation. Iodine refers to vital trace elements, is an integral part of thyroid hormones. Thyroid hormones are involved in the regulation of metabolic processes in the body: protein, fat, carbohydrate and energy are necessary for the development of all organs and systems, regulate the activity of the brain, nervous and cardiovascular systems, genitals and mammary glands, play an important role in growth and development the child, the formation of his intellectual abilities. Iodine is especially necessary for children, adolescents, women during pregnancy and during lactation (breastfeeding).

Iodine balance, being a source of iodine, replenishes its deficiency in the body, prevents the development of iodine deficiency diseases, prevents the development of goiter, associated with a lack of iodine in food. Normalizes the size of the thyroid gland in newborns, children, adolescents and adults.

Pharmacokinetics

Absorption and distribution of

When taken orally, the drug is almost completely absorbed in the small intestine.

The average Vd for healthy people is approximately 23 L (38% of body weight). The plasma concentration of iodine in the normal range is from 0.001 to 0.005 μg / ml. It accumulates in the thyroid gland, salivary glands, mammary glands and tissues of the stomach. The concentration in saliva, gastric juice and breast milk is approximately 30 times higher than in blood plasma.

Excretion

Excreted in urine. The concentration of iodine in urine relative to creatinine (μg / g) is an indicator of its intake in the body.

Contraindications

Iodine balance should not be taken with hypothyroidism, unless the development of the latter is caused by severe iodine deficiency.

Prescribing should be avoided with iodine therapy if there is or is suspected of thyroid cancer.

The drug contains lactose monohydrate, therefore, it is not recommended that patients with rare hereditary diseases associated with galactose intolerance, lactase deficiency or glucose-galactose malabsorption are not recommended.

Use during pregnancy and lactation

During pregnancy and during breastfeeding, the demand for iodine increases, so it is especially important to use the drug Iodbalance ® in sufficient doses to ensure adequate intake of iodine in the body. The drug crosses the placental barrier.

The drug is excreted in breast milk. If a lactating woman is taking Iodine Balance ®, then an additional prescription of the drug to breast-fed infants is not required.

The use of the drug during pregnancy and during breastfeeding is possible only in recommended doses.

When conducting therapy, the amount of iodine supplied with food must be considered.

Dosage and administration of

When determining the required dose of Iodbalance ®, regional and individual characteristics of the intake of iodine with food should be taken into account. This is especially important when prescribing the drug to newborns and children under the age of 4 years.

For the prevention of iodine deficiency diseases, a preparation is prescribed for newborns and children at a rate of 50-100 mcg iodine / day (1 / 2-1 tablets. Iodbalance ® 100 μg) for adolescents and adults – 100-200 mcg iodine / day (1 tab. 100 mcg or 1 tab. Of the drug Iodbalance ® 200 mcg) during pregnancy and lactation (breastfeeding) – 100-200 mcg iodine / day (1 tab. the drug Iodbalance ® 100 mcg or 1 tab. the drug Iodbalance ® 200 mcg).

For the prevention of relapse of goiter after surgical removal or at the end of the course of treatment of goiter with thyroid hormone preparations – 100-200 micrograms of iodine daily (1 tablet of the drug Yodbalans ® 100 μg or 1 tablet of the drug Iodbalance 200 μg).

In the treatment of euthyroid goiter, 100-200 micrograms of iodine / day are prescribed for newborns and children (1 tab. of the drug Iodbalance ® 100 micrograms or 1 tab. of the drug Iodbalance ® 200 micrograms). Adolescents, adult young patients are prescribed 200 mcg of iodine / day (2 tablets of the drug Iodbalance ® 100 μg or 1 tablet of the drug Iodbalance ® 200 μg).

The daily dose should be taken in 1 dose, after eating, drinking plenty of fluids. For newborns and children under the age of 3, it is recommended to dissolve the tablet in a small amount (1 tablespoon) of boiled water at room temperature.

The use of the drug for prophylactic purposes is usually carried out for several months or years, often throughout life.

For the treatment of goiter in newborns, in most cases, 2-4 weeks in children, adolescents and adults usually takes 6-12 months or more. The duration of treatment is determined by the doctor individually.

Side effects

Allergic reactions: rarely – skin rash, Quincke edema.

Drug Interaction

The effectiveness of treatment with antithyroid drugs with the concomitant administration of iodine is reduced.

Potassium perchlorate inhibits the absorption of iodine by the thyroid gland.

The use of iodine at high doses and the simultaneous administration of potassium-sparing diuretics can lead to the development of hyperkalemia.

The simultaneous administration of high-dose iodine and lithium preparations contributes to the development of goiter and hypothyroidism.

Thyroid absorption of iodine and its metabolism are stimulated by TSH.

Overdose

Symptoms: the administration of the drug at a dose of more than 150 μg / day in patients with thyroid foci of functional autonomy, the development of iodine-induced hyperthyroidism is possible.

When treated with iodine at very high doses (> 1000 g / day), iodine-induced goiter and hypothyroidism may develop in some cases.

Chronic overdose can lead to the development of iodism – a metallic taste in the mouth, swelling and inflammation of the mucous membranes (rhinitis, conjunctivitis, gastroenteritis, bronchitis), acne, dermatitis, swelling of salivary glands, fever, irritation.

Treatment: In chronic overdose, it is recommended to discontinue the drug. In the development of iodine-induced hyperthyroidism, it is recommended to discontinue use of the drug and prescribe therapy with antithyroid agents. In severe cases, intensive care, plasmapheresis, or thyroidectomy should be performed. When developing hypothyroidism, it is recommended to discontinue use of the drug and to prescribe therapy with iodine-containing thyroid hormones.
