Pharmacological action
Prednisolone ointment has a pronounced local anti-inflammatory, anti-allergic, antiexudative effect. The anti-inflammatory effect of prednisone in the skin and other tissues is achieved mainly with the participation of cytosolic glucocorticosteroid receptors. The hormone-receptor complex, penetrating the nucleus of the target cell of the skin (keratinocytes, fibroblasts, lymphocytes), enhances the expression of genes encoding the synthesis of lipocortins, which inhibit phospholipase A2 and reduce the synthesis of metabolic products of arachidonic acid – cyclic endoperoxides, prostaglandins and thromboxane. The antiproliferative effect of prednisone is associated with inhibition of the synthesis of nucleic acids (primarily DNA) in the cells of the basal layer of the epidermis and fibroblasts of the dermis. The antiallergic effect of the drug is due to a decrease in the number of basophils, direct inhibition of the synthesis and secretion of biologically active substances.
Pharmacokinetics
When applied topically, prednisone is absorbed and the active substance enters the bloodstream. In plasma, 90% of prednisolone is bound (with transcortin and albumin). Biotransformed by oxidation, mainly in the liver, oxidized forms are glucuronidized or sulfated. It is excreted in urine and feces in the form of metabolites, partially in unchanged form. Penetrates through the placental barrier and is found in small amounts in breast milk.
Indications
Urticaria, atopic dermatitis, diffuse neurodermatitis, simple chronic lichen (limited neurodermatitis), eczema, seborrheic dermatitis, discoid lupus erythematosus, simple and allergic dermatitis, toxidermia, erythroderma, psoricitis leoparditis, periorititis bile, psoricitis leoparditis, aloperatitis, leoparditis, leoparditis, leoparditis, aloperatitis, periorititis, aloperatitis, leoparditis, alopecia pylitonitis, perioceritis, aloperatitis, leprosy keloid scars, sciatica.
Contraindications
Bacterial, viral, fungal skin diseases, skin manifestations of syphilis, skin tuberculosis, skin tumors, acne vulgaris, rosacea (possibly exacerbation of the disease), pregnancy.
Use during pregnancy and lactation
Contraindicated.
Composition of
100 g of ointment contains:
Active ingredient: prednisolone – 0.5 g
Excipients: glycerol (distilled glycerin), medical petrolatum, stearic acid, emulsifier No. 1, methyl parahydroxybenzo benzoate (water), propanohydroxybenzo benzoate (n), peeled.
Dosage and administration
Ointment is applied in a thin layer to the affected skin 1-3 times a day. The duration of the course of treatment depends on the nature of the disease and the effectiveness of therapy, usually 6-14 days. If necessary, treatment can be continued.
Occlusive dressings can be used on limited foci to enhance the effect.
When applying the ointment in children from 1 year of age and older, it is necessary to limit the total duration of treatment and exclude activities, leading to increased resorption and absorption of the steroid (warming, fixing and occlusive dressings).
Side effects
Steroid acne, purpura, telangiectasia, burning and itching of the skin, irritation and dryness of the skin with prolonged use and / or when applied to large surfaces may cause systemic side effects, the development of hypercorticism (in these cases, the ointment is canceled) with prolonged use ointments may also develop secondary infectious skin lesions, atrophic changes, hypertrichosis.
Drug Interaction
Prednisolone ointment enhances the anticoagulant effect of anticoagulants. Increases the likelihood of bleeding with salicylate. Increases the possibility of electrolyte exchange disturbances while taking diuretics. Increases the risk of cardiac glycosides intoxication. Reduces the hypoglycemic effect of anti-diabetic drugs and reduces the antibacterial activity of rifampicin. It is not recommended to combine with antacid drugs.
