Pharmacological action

Pyrantel – anthelmintic.

Blocks neuromuscular transmission in sensitive helminths.

Pharmacodynamics

Acts on helminths in the early phase of development and on mature forms, does not affect larvae in the migration stage. Active against Enterobius vermicularis, Ascaris lumbricoides, Ancylostoma duodenale, Necator americanus, Trichostrongylus orientalis, Trichostrongylus colubriformis.

Pharmacokinetics

Practically not absorbed in the digestive tract. After oral administration in a single dose of 10 mg / kg body weight, Cmax is 0.005 “0.13 μg / ml and is achieved within 1-3 hours. It is partially metabolized in the liver to N-methyl-1,3-propanediamine. It is excreted with feces (50% of the dose taken – unchanged) and with urine (about 7% – unchanged and as a metabolite).

Indications

Enterobiosis, ascariasis, hookworm infection, necatorosis.

Contraindications

Hypersensitivity.

Pregnancy and lactation

Adequate and strictly controlled safety studies of the use of Pyrantel during pregnancy and lactation (breastfeeding) have not been conducted. The use of the drug during these periods is possible only if the intended benefit to the mother outweighs the potential risk to the fetus or child.

In experimental studies, no negative effect of Pyrantel on the fetus was found.

Special instructions

For enterobiosis, simultaneous treatment of cohabiting individuals should be carried out. After completion of treatment, it is necessary to conduct a control study of feces on the eggs of the worm.

Dosage and Administration

Inside. The dose and frequency of administration are set individually, depending on the indications, age and body weight of the patient.

In case of ascariasis and enterobiasis, it is prescribed once at the rate of 10 mg / kg: for adults and children over 12 years old – 750 mg (for body weight over 75 kg – 1 g) for children from 6 months to 2 years old – 125 mg, 2-6 years old – 250 mg, 6-12 years – 500 mg. In order to avoid self-invasion, it is recommended that the course be repeated 3 weeks after the first dose.

With hookworm infection – 10 mg / kg in 1 dose for 3 days.

In case of necatorosis (severe forms) – 20 mg / kg / day for 2 days.

With isolated ascariasis, 5 mg / kg once.

Chew tablets before swallowing.

Side effects

From the nervous system and sensory organs: rarely – headache, dizziness, drowsiness, insomnia, weakness in some cases – hearing impairment, hallucinations, confusion, paresthesia.

From the digestive tract: nausea, vomiting, diarrhea, stomach pain rarely – increased activity of hepatic transaminases.

Other: skin rash, fever.

Drug Interactions

With simultaneous use with piperazine, the action of Pyrantel is weakened.

Pharmacokinetic interaction

With the simultaneous use of pyrantel may increase the concentration of theophylline in blood plasma.

Overdose

No overdose has been reported.

In case of overdose, symptomatic treatment should be taken.
