Pharmacological action of
The mechanism of action of diclofenac is a basic enzyme inhibitor of cycloid oxidase, which play a major role in the pathogenesis of inflammation, pain, and fever. Thiamine (vitamin B1) in the human body as a result of phosphorylation processes turns into cocarboxylase, which is a coenzyme of many enzyme reactions. Vitamin B1 plays an important role in carbohydrate, protein and fat metabolism. Actively involved in the processes of nervous excitation in synapses. Pyridoxine (vitamin B6) is necessary for the normal functioning of the central and peripheral nervous system. In phosphorylated form, it is a coenzyme in the metabolism of amino acids (decarboxylation, transamination). It acts as a coenzyme of the most important enzymes that act in nerve tissues. Cyanocobalamin (vitamin B12) is necessary for normal blood formation and maturation of red blood cells, and is also involved in a number of biochemical reactions that ensure the vital activity of the body – in the transfer of methyl groups, in the synthesis of nucleic acids, protein, in the exchange of amino acids, carbohydrates, lipids.
Indications
Indications for use of the drug Clodifen Neuro are:
– pain with inflammation of a non-rheumatic nature (after injuries, surgical and dental interventions for gynecological diseases – primary algomenorrhea, adnexitis in inflammatory diseases of the ENT organs – pharyngitis, tonsillitis, tonsillitis) – inflammatory and degenerative diseases of the spine and joints (rheumatoid arthritis, ankylosing spondylitis – ankylosing spondylitis, spondylarthritis,
spondylarthrosis, osteoarthritis)
– neuritis and neuralgia (cervical syndrome, lumbago, sciatica)
– acute gouty arthritis
– rheumatic diseases of the periarticular soft tissue.
Contraindications
Contraindications to the use of the drug Clodifen Neuro are: hypersensitivity to the components of the drug gastric ulcer and 12 duodenal ulcer III trimester of pregnancy children under 12 years of age.
Special instructions
The use of Clodifen Neuro is possible only under strict medical supervision in the following conditions: induced porphyria of I and II trimesters of pregnancy during lactation.
Careful medical supervision is required: in case of diseases of the gastrointestinal tract and / or inflammatory and ulcerative diseases of the gastrointestinal tract (ulcerative colitis, Crohn’s disease) in the anamnesis in patients with renal lesions with high blood pressure and / or heart failure in elderly patients immediately after heavy surgical intervention.
With great caution, Clodifen Neuro can be used in patients with asthma, allergic (hay) runny nose, nasal mucosa polyps, as well as in patients with hypersensitivity to NSAIDs due to the risk of an asthma attack, Quincke’s edema or urticaria.
During the treatment period, care must be taken when driving vehicles and engaging in other potentially hazardous activities that require increased concentration of attention and speed of psychomotor reactions.
Composition
1 capsule contains: Active substances: diclofenac sodium – 50 mg, pyridoxine hydrochloride – 50 mg, thiamine hydrochloride (vit. B1) – 50 mg, cyanocobalamin – 0.25 mg.
Excipients: lysine hydrochloride – 110.75 mg, microcrystalline cellulose – 150 mg, anhydrous colloidal silicon dioxide – 4 mg, magnesium stearate – 5 mg.
Composition of hard gelatin capsule No. 0:
case – titanium dioxide – 0.944 mg, red iron oxide – 0.177 mg, crimson dye (Ponceau 4R) – 0.0527 mg, diamond black dye – 0.0034 mg, purified water – 8.555 mg, gelatin – 49.265 mg.
cap – titanium dioxide – 0.74 mg, yellow iron oxide – 0.0925 mg, crimson dye (Ponceau 4R) – 0.0006 mg, purified water – 5.365 mg, gelatin – 30.8025 mg.
Dosage and administration
Clodifen Neuro capsules should be taken orally with food, without chewing and drinking plenty of fluids.
For adults, the drug is prescribed 1 capsule 3 times a day at the beginning of treatment, as a maintenance dose, 1 capsule 1-2 times a day. The duration of therapy depends on the nature and severity of the disease.
Side effects
Side effects depend on the individual sensitivity, the size of the dose taken and the duration of treatment.
From the gastrointestinal tract: nausea, vomiting, diarrhea can sometimes occur, rarely – lack of appetite, abdominal cramps, feeling of fullness of the stomach, the development of ulcers in the gastrointestinal tract, which in some cases leads to the appearance of blood impurities in the stool.
From the side of the central nervous system: sometimes headaches, dizziness may occur, rarely sleep disturbances, irritability.
Dermatological reactions: sometimes itching, rash, urticaria are observed.
From the side of the kidneys: in some cases, there were impaired renal function, proteinuria, hematuria and kidney damage (interstitial nephritis, nephrotic syndrome), therefore, regular monitoring of renal function is necessary.
From the liver: sometimes there is an increase in the level of transaminases in serum. With prolonged use, regular monitoring of liver function is recommended.
From the hematopoietic system: in rare cases, anemia, leukopenia, agranulocytosis, thrombocytopenia were observed.
Other: hypersensitivity reactions to diclofenac may occur in the form of swelling of the face, tongue, larynx, rapid heartbeat, increased blood pressure.
Overdose
Symptoms of an overdose of Clodifen Neuro: vomiting, dizziness, headache, shortness of breath, blurred consciousness in children – myoclonic seizures, nausea, abdominal pain, bleeding, impaired liver and kidney function.
Treatment: gastric lavage, the introduction of activated carbon. Conduct symptomatic therapy, forced diuresis. Hemodialysis is ineffective.
