Pharmacological action

Pharmgroup: glucocorticosteroid for topical use.

Pharmaceuticals: Lokoid is a synthetic non-halogenated glucocorticosteroid for external use. The esterification of the hydrocortisone molecule with the remainder of butyric acid (butyrate) allowed a radical increase in the activity of the drug compared to native hydrocortisone. Lokoid has a rapidly onset anti-inflammatory, decongestant, antipruritic effect.

The use of Lokoid in recommended doses does not cause suppression of the hypothalamic-pituitary-adrenal system. Although the use of large doses of the drug for a long time, especially when using occlusive dressings, can lead to an increase in the content of cortisol in the blood plasma, this is usually not accompanied by a decrease in the reactivity of the pituitary-adrenal system, and drug withdrawal leads to a quick normalization of cortisol production.

Pharmacokinetics: After application, the active substance accumulates in the epidermis, mainly in the granular layer, systemic absorption is negligible. A small amount of hydrocortisone 17-butyrate is absorbed into the systemic circulation unchanged.

Most hydrocortisone 17-butyrate is metabolized to hydrocortisone and other metabolites directly in the epidermis and subsequently in the liver.

Metabolites and a small portion of unchanged hydrocortisone 17-butyrate are excreted by the kidneys and through the intestines.

Indications

Superficial, uninfected, sensitive to local glucocorticosteroids skin diseases: – dermatitis, including atopic, contact, seborrheic

– eczema

– psoriasis.

Contraindications

– hypersensitivity to hydrocortisone or auxiliary components of the drug

– bacterial skin infections (strepto- and staphyloderma, gram-negative folliculitis, etc.)

– viral infections of the skin (herpes simplex, chickenpox, etc. )

– fungal skin infections

– tuberculosis and syphilitic skin lesions

– parasitic skin infections

– neoplastic skin changes (benign and malignant tumors)

Special instructions

May be used in children from 6 months of age. In cases where the drug is used in children, on the face or under occlusive dressings, the duration of the course of treatment should be reduced.

When used in children, the surface area of ​​the skin on which the drug is applied should not exceed 20% of the total.

In childhood, suppression of adrenal cortex function may develop faster. In addition, a decrease in growth hormone excretion may be observed. When using the drug for a long time, it is necessary to monitor body weight, height, plasma cortisol level. In a study in children who received 30-60 g of Lokoid per week for 4 weeks in comparison with 1% hydrocortisone ointment, not one child showed dysfunction of the adrenal cortex, with differences in hydrocortisone 17-butyrate in this indicator from usual hydrocortisone was not observed.

Lokoid should not be applied to the periorbital area due to the risk of developing glaucoma. With a systematic exposure to the conjunctiva, there is a risk of an increase in intraocular pressure.

When localizing the process on the scalp and exudative inflammation, it is preferable to use Lokoid in the form of a Crelo.

In cases of resistant disease, for example, when localizing dense psoriatic plaques on the elbow areas, knees, severe lichenification, dryness, it is better to give preference to Lokoid in the form of ointment or Lipocream, if necessary, under occlusive dressings.

The drug is not advisable to use in the presence of atrophic skin changes. The risk of local and systemic side effects increases when applied to large areas of the lesion, prolonged use, the use of occlusion and in childhood.

If there is no effect for 2 weeks of continuous treatment, the diagnosis should be clarified.

As with any glucocorticosteroid drug, it is advisable to limit the duration of use and the course dose to the minimum sufficient to stop the skin process.

Influence on the ability to drive vehicles and work with mechanisms: There is no data on the influence of Lokoid on the ability to drive vehicles and work with mechanisms.

Composition

Excipients: cetostearyl alcohol 72 mg, macrogol 25 cetostearyl ether 18 mg, liquid paraffin 60 mg, soft white paraffin 150 mg, propyl parahydroxybenzoate 1 mg, butyl parahydroxybenzoate 0.5 mg, anhydrous citric acid 4.2 mg, anhydrous sodium citrate 2.8 mg, purified water up to 1 g.

Dosage and Administration

Externally.

Lokoid cream is preferably used for acute and subacute skin processes (without soaking).

The drug is applied to the affected areas of the skin with a thin layer 1-3 times a day. To improve penetration, the drug is applied with light massaging movements. With the appearance of positive dynamics, the frequency of use of the drug can be reduced to 2-3 times a week.

The dose of the drug used during the week should not exceed 30-60 g.

Side effects

Local reactions: skin irritation, in rare cases, other local reactions characteristic of external steroid therapy.

Systemic reactions: with prolonged use, application to large surfaces and / or the use of occlusion, side effects characteristic of systemic glucocorticosteroids may be observed. The risk of local and systemic side effects when using Lokoid is lower than when using fluorinated steroids.

Overdose

No data on overdose. With prolonged use of the drug in high doses, it is possible to enhance the side effects described above. When symptoms of hypercorticism appear, the drug should be discontinued.
