Pharmacological action
Azathioprine is an immunosuppressive drug. Suppresses the reaction of tissue incompatibility.
Indications
Prevention of transplant rejection reaction (as part of combination therapy). Severe rheumatoid arthritis, systemic lupus erythematosus, dermatomyositis, periarteritis nodosa, pemphigus vulgaris, idiopathic thrombocytopenic purpura, acquired hemolytic anemia, pyoderma gangrenous, chronic hepatitis.
Contraindications
Pregnancy, hypersensitivity to azathioprine and / or mercaptopurine.
Pregnancy and lactation
Azathioprine is contraindicated in pregnancy. During treatment, women of childbearing age should use reliable methods of contraception.
If necessary, use during lactation should stop breastfeeding.
In experimental studies, the teratogenic, embryotoxic and carcinogenic effects of azathioprine have been established.
Special instructions
During the first 8 weeks of treatment, weekly monitoring of the picture of peripheral blood is shown, in the subsequent 1-2 times a month, as well as periodic monitoring of hepatic transaminases, alkaline phosphatase activity and bilirubin levels.
In cases of impaired renal and / or liver function, as well as with concomitant use of allopurinol, azathioprine should be used in lower doses.
Composition
1 tablet contains 50 mg of azathioprine.
Dosage and administration
Set individually, taking into account the nosological form and severity of the disease, the dosage of the simultaneously prescribed drugs, hematological tolerance.
To prevent the transplant rejection reaction, it is prescribed in a loading dose of 5 mg / kg / day in 2-3 doses for 1-2 months, then they switch to a maintenance dose of 1 to 4 mg / kg / day for several years. In other cases, the daily dose is 1-2.5 mg / kg. The treatment is long.
Side effects of
From the hematopoietic system: myelodepression (leukopenia, thrombocytopenia, anemia) may develop secondary infections, megaloblastic erythropoiesis and macrocytosis in isolated cases – hemolytic anemia.
From the digestive system: nausea, vomiting, anorexia, cholestatic hepatitis are possible in transplant recipients, pancreatitis, erosive and ulcerative lesions and bleeding from the gastrointestinal tract, necrosis and perforation of the intestine are noted.
Allergic reactions: skin rash, arthralgia, myalgia, drug fever are possible.
Other: in isolated cases – acute renal failure, acute pulmonary disease, meningeal reactions.
Drug Interactions
With simultaneous use with co-trimoxazole, the myelotoxic effect of azathioprine may be enhanced. Cases of severe leukopenia with simultaneous use with ACE inhibitors are described.
