Pharmacological action
Carboxy belongs to the group of pyridinium cholinesterase reactivators. It has a pronounced antidote effect in the treatment of lesions with organophosphorus compounds (FOS).
Timely administration of carboxyme significantly accelerates the restoration of cholinesterase activity inhibited by FOS.
Also has a positive effect on some other manifestations of poisoning (reduces spasm of the bronchi, motor disorders, etc.).
Carboxyme has low toxicity, practically has no anticholinergic effect.
Indications
the appearance of the initial signs of FOS poisoning (agitation, miosis, salivation, etc. ) in the provision of medical care, starting with the
health center, the detection in persons under observation after staying in the infection site of a decrease in cholinesterase activity.
Carboxyme is used against the background of antidotes (atropine, Athens, P-10M) and other drugs used in the treatment of FOS poisoning. After the administration of the therapeutic antidote, carboxy is administered after 1-2 hours.
Contraindications
There are no contraindications for the use of carboxim if there are symptoms of FOS intoxication.
Composition
1 ml of solution for intramuscular injection contains: active substance: carboxim 150 mg,
excipients: citric acid, sodium hydroxide, sodium chloride, disodium edetate (Trilon B), water for injection.
Dosage and administration of
Intramuscularly 1 ml. The total dose of carboxyme used in relief depends on the severity of the lesion.
For lesions of FOS of mild degree (anxiety, miosis, salivation, sweating) after administration of cholinolytic drugs of the type of action, carboxy is used in one therapeutic dose. In some cases, re-administration of the drug in the same dose is possible.
For lesions of FOS of moderate severity (excitement, myofibrillation, bronchospasm, cyanosis, tachycardia, less often bradycardia), as a rule, twice the use of carboxyme in a single therapeutic dose with an interval of 2-3 hours will be required.
In cases of severe FOS lesions (convulsions, coma, respiratory and cardiovascular failure), the dose of carboxyme may be increased to 3-4 ml. This dose is administered in 1 ml of the drug at intervals of 1-2 hours. Treatment of affected FOS with carboxyme in all cases is carried out with systematic
monitoring of cholinesterase activity.
Side effects
At the recommended dose, carboxyme does not cause side effects.
However, when using large doses of carboxyme, there may be a deterioration in well-being, muscle weakness, dizziness, headache, nausea, and tachycardia.
Transient paresthesia may occur (mainly in the face). On the ECG, phenomena of intracardiac conduction disturbance and an increase in systolic rate may be observed.
The indicated phenomena soon pass and do not require additional treatment.
