Pharmacological action

The active ingredient in Norkolut is norethisterone, which is mainly a progestogen-type norsteroid. Compared with other known gestagens, the drug, although it does not belong to the most active gestagens, yet its properties are unusually favorable for therapy.

With the so-called delay of mensis test (delaying menstruation), which is used to determine the effectiveness of gestagens, the drug exhibits the properties of weak gestagens. The compound turned out to be a medium-intensity progestive steroid when tested according to Hammerstein, as well as in the Clauberg test, which measures the dose needed to transform the endometrium.

In contrast to many gestagen compounds, norethisterone has a pronounced antiestrogenic character, and unlike other derivatives of nortestosterone in the body, only a small part of the compound is converted to estrogen.

At the same time, Paulsen under the influence of norethisterone in men observed gynecomastia, and in women an increase in mammary glands. In addition to the main gestagen action, the weak androgenic property of the compound was also determined. According to clinical and pharmacological studies, norethisterone blocks the production of follicle-stimulating hormone (FSH) and corpus luteum hormone (LH), and reduces the synthesis of steroids in the corpus luteum.

Norethisterone, which has mainly gestagenic properties, exhibits significant antiestrogenic and minimal estrogenic and androgenic effects, can be very widely used in obstetrics and gynecology.

Indications

Premenstrual syndrome, mastodynia, cycle disorders, accompanied by a shortened secretion period, dysfunctional uterine bleeding, cystic glandular endometrial hyperplasia, endometriosis, cessation, and cessation menopause, in case of individual need, oral contraception.

Contraindications

Predisposition to thrombosis, hepatitis, liver function disorders, malignant tumors of the mammary gland and genitals.

Dosage and administration of

In case of premenstrual syndrome, mastodynia, and irregular cycles, 1-2 tablets are prescribed starting from the 16th day of the cycle to the 25th day of the cycle, this dosage can be supplemented with estrogen.

For dysfunctional uterine bleeding, cystic glandular endometrial hyperplasia (if the functional origin of the bleeding is confirmed by histological analysis, carried out in a 6-month period) in order to stop the bleeding, 1-2 tablets per day are prescribed for 6-12 days. To prevent relapse from the 16th day of the cycle to the 25th day of the cycle, 1-2 tablets are prescribed per day, usually together with estrogen. In the case of a malignant process, Norkolut therapy is not used.

In case of endometriosis, adenomyosis, starting from the 5th day and ending with the 25th day of the cycle, apply 1 tablet per day for 6 months or continuously from the 5th day of the cycle 1/2 tablet per day, increasing the dose by 1 / 2 tablets every 2-3 weeks, for 4-6 months.

To stop lactation, take 1-3 tablets on days 1-3, 3 tablets on days 4-7, and 2 tablets on days 8-10. For menopausal symptoms, 1 tablet per day is prescribed continuously (for months), or, in case of failure, used in combination with 25-50 micrograms of ethinyl estradiol. For oral contraception, 1 / 4-1 / 2 tablets of Norkolut and 0.05 mg of ethinyl estradiol are used starting from the 5th day of the cycle for 21 days, then after a 7-day break the course is repeated. If necessary, a 2-phase contraceptive dosage is prescribed from the 5th day of the cycle for 10 days for 1/4 tablet of Norkolut, then for 11 days 1/2 tablet of Norkolut and 0.05 mg of ethinyl estradiol.

To delay menstruation, treatment begins 8 days before the expected menstruation and lasts for 12 days: at the same time, 2 Norkolut tablets are prescribed per day, this dosage provides approximately a 7-8-day delay in menstruation.

Side effects

Rarely seen in subsequent cycles: nausea, gastrointestinal complaints, extraordinary bleeding, sometimes swelling, allergic skin reaction, paresthesia, weight loss, fatigue.

Drug interactions

Drugs that activate microsomal liver enzymes (barbiturates, etc.), accelerate metabolism and weaken the effect, on the contrary, inhibitors (cimetidine, etc.) strengthen it.
