Pharmacological action
Pimobendan, which is part of Vetmedin, has a positive inotropic effect and a pronounced vasodilating effect.
Unlike cardiac glycosides, the positive inotropic effect of the drug is due to two mechanisms: increased sensitivity to calcium of the fibers of the heart muscle and inhibition of phosphodiesterase.
The vasodilating effect is associated with the inhibition of phosphodiesterase III activity. Due to the presence of a positive inotropic and vasodilating effect in heart failure, the drug increases the strength of heart contractions and reduces both preload and afterload.
After oral administration, pimobendan is rapidly absorbed into the blood, and its bioavailability is 60-63%. The half-life of pimobendan in blood plasma is 0.4 В± 0.1 hours, and the half-life of its active metabolite is 2 В± 0.3 hours. Most of the drug is excreted from animals with feces.
Vetmedin by its degree of exposure to the body belongs to low-hazard substances (hazard class 4 according to GOST 12.1.007-76), does not have a sensitizing, embryotoxic, teratogenic and mutagenic effect.
Indications
Assign for the treatment of cardiovascular disease in dogs. Indication for use is heart failure caused by dilated cardiomyopathy, bicuspid or tricuspid valve insufficiency, which is accompanied by characteristic symptoms: cough, shortness of breath, decreased activity, or ascites.
Contraindications
It is forbidden to use Vetmedin: with hypertrophic cardiomyopathy and clinical conditions, in which the cardiac output cannot be increased due to functional or anatomical features (for example, aortic stenosis), use Vetmedin in dogs with severe liver damage, since the drug is predominantly metabolized in the liver, use Vetmedin capsules together with calcium channel blockers and beta-blocker propranolol, since these substances weaken the effect of pimobendan.
Recommendations for use
Vetmedin is administered orally. The daily dose is 0.5 mg of active substance per 1 kg of body weight. The dosage can range from 0.2 mg to 0.6 mg of pimobendan per 1 kg of body weight per day, according to the recommendation of a veterinarian, and also depending on the severity of the course of the disease and the condition of the animal.

In case of congestive heart failure, Vetmedin is used in animals for life with the selection of an individual dose.
Vetmedin is given to the animal an hour before feeding.
In case of drug overdose, an animal may experience vomiting, apathy, tachycardia, heart murmur, or decreased pressure. In case of an overdose, stop giving the drug and consult a veterinarian. It is recommended that the immediate non-specific decontamination of the drug, including the possible use of activated carbon and / or maintaining the water-electrolyte balance, taking into account the condition of the animal.
Peculiarities of the pharmacological effect on the animal organism during the first use of the drug and during its withdrawal were not revealed.
If the interval between drug administrations is accidentally increased, it should be used as soon as possible in the prescribed dosage.
Special instructions
The drug is prescribed for puppy and lactating bitches if the potential risk to the mother outweighs the risk to the fetus and puppies. The drug is used for unproductive animals.
Composition
Vetmedin tablets / capsules contain 10 mg of active ingredient, pimobendan, and auxiliary components: anhydrous citric acid – 413 mg, microcrystalline cellulose – 80 mg, anhydrous colloidal silicon – 22 mg, povidone – 8 mg and magnesium stearate – 3 mg .
Side effects
Side effects and complications when using Vetmedin in accordance with these instructions for use, as a rule, are not observed. In rare cases, diarrhea, lethargy, anorexia may occur. Also, some animals may experience vomiting, tachycardia, which can be eliminated by reducing the dose of the drug.
